Job Description
ARC Programs is currently accepting applications for Intake and Care Coordinators to join the Family Connection Centre team.


Intake and Care Coordinators are the primary care coordinators within the Family Connection Centre (FCC).


Intake and Care Coordinators provide intake and service coordination functions for parents / caregivers and their children or youth with support needs, who are accessing services through the FCC. Intake and Care Coordinators also may provide counselling, support, and educational services on an individual, family or group-basis.


Hours / Schedule >


Full-Time, Permanent Averaging 37.5-hours per-week


Responsibilities >


Intake And Care Coordinators Will Provide Individualized Service Planning Support For Families, To Address Identified Needs And Coordinate Access To FCC Services


  • Implement required case management and clinical records
  • Other responsibilities include:
  • Develop a goal-oriented Individualized Service Plan (ISP) to address service needs; which may include services to be provided by the Intake and Care Coordinator, and / or other practitioners of the FCC.
  • Conduct brief screening and assessments to identify the specific service needs of each child, youth and/or family.
  • Provide needs-based screening and developmental monitoring throughout service engagement, and update service goals as necessary in collaboration with other FCC practitioners.
  • Connect the child, youth or family and support the transition between relevant FCC services.
  • Assist families as needed with appointment reminders and coordination of services, needs identification, advocacy and organizing consistent Integrated Case Management meetings with child / youth and family’s interdisciplinary team.
  • Provide information regarding child development, available services within the FCC, and other available resources in the community.
  • Building relationships and connections within the community to improve service integration.
  • Appling culturally safe and agile, trauma informed, relational and person / family-centered modalities.
  • Remain connected with regular individual meetings as the family’s consistent point of contact throughout their service.


Qualifications >


  • Candidates must possess a valid Driver's License, and a sound personal vehicle to provide outreach support, and transportation for families when required.
  • Successful applicants will be subject to a screening process, including a criminal record search, to confirm suitability for the position.
  • A bachelor’s degree in a Social Services or Human Services discipline (ie: BSW, CYC, BA in Psychology etc.) with a minimum of 2-years relevant experience is preferred; although other levels of qualifications including position-specific experience, knowledge and skills may be considered.

  • Experience working as part of a multi-disciplinary team.
  • Knowledge and demonstrated understanding of strengths-based family systems and attachment-based support.
  • Ability to function effectively both independently and as part of a team, in a fast-paced working environment.
  • Ability to establish trusting relationships with children, youth, and families.
  • Well-versed in child development, developmental delays, neurodiversity, ASD, FASD and complex support needs.
  • Familiar with service coordination, including IHAN assessments and recommended interventions.


Wage / Benefits >


$ 4,385- $4,790 per month, based on relevant experience and education.


Upon successful completion of a probationary period, full-time employees (minimum 20-hrs per week) are eligible for a full benefit package (including dental plan, vision coverage, life insurance, extended medical coverage, long-term disability plan, and RRSP contributions).


New full-time employees are eligible for 3-weeks paid vacation to start, as well as paid sick days as described in agency policy.
