Health Promoter (Pft) (Cupe 103.23)

Scope of Position:

Assess, promote, protect and enhance health and prevention of disease and injuries. Use population-health approaches, based on health promotion theories and evidence. Plan, implement, and evaluate health promotion strategies. Meet and adhere to the Public Health Core Competencies as defined by the Public Health Agency of Canada.

Position Description:

• Engage with colleagues in a systematic approach to quality improvement, including the definition of the purpose and scope; development, testing and implementation of change ideas; and measurement of reach and impact
• Participate as a member of an interdisciplinary team within the Health Unit
• Perform other duties as assigned
• Contribute to the identification and prioritization of areas for improvement within the six dimensions of quality (safe, effective, efficient, timely, person-centred and equitable)
• Review, develop, and/or acquire health promotion resources
• Demonstrates knowledge and skills necessary for health promotion practice in the areas of population-health promotion, determinants of health and health equity
• Facilitate community mobilization and build community capacity around shared health priorities
• Conduct situational assessments including, analysis of data, evidence and environmental scan findings
• Develop, implement, evaluate and oversee evidence-based health promotion programs/plans which meet Ontario Public Health standards and requirements
• Contribute to health-related policy development and advocacy that reflects community needs
• Apply social marketing and other communication principles to the development, implementation and evaluation of health communication strategies

Requirements

Knowledge and Experience:

• Four year University degree in a related discipline (Health Education / Health Promotion / Sciences), or equivalent
• One year, up to and including two years’ experience to be familiar with scope of position

Skills and Abilities:

• Travel required within Haldimand and Norfolk Counties.
• Computer literacy and proficiency in corporate standard software
• Be proactive in adhering to Health and Safety practices
• Proficient oral and written communication skills required
