Demonstrates knowledge and skills necessary for health promotion practice in the areas of population-health promotion, determinants of health and health equity

Develop, implement, evaluate and oversee evidence-based health promotion programs/plans which meet Ontario Public Health standards and requirements

Four year University degree in a related discipline (Health Education / Health Promotion / Sciences), or equivalent

One year, up to and including two years’ experience to be familiar with scope of position

Proficient oral and written communication skills required

Assess, promote, protect and enhance health and prevention of disease and injuries.

High school diploma with one-year of relevant post-secondary education or an equivalent combination of education and experience

Perform data entry and maintain statistical records

Able to use a database, operate computers and is skilled in using Microsoft Office (Outlook, Word and Excel)

Strong interpersonal skills and demonstrated client service focus

General knowledge of the practice of law is considered an asset

Type and review letters, documents and other materials from instructions or precedents

Alzheimer’s Care *Previous experience not necessary but is an asset* *Skills training and ongoing training provided by Home Instead*

Requirements To Be a CARE Professional

Light housekeeping tasks and meal preparation

Ability to treat and care for community members and their property with dignity and respect

Drive and deliver vehicles locally or out of area as needed, following all rules of the road

Deliver customers and vehicles to appropriate destination in a safe and courteous manner

Ride with or follow another driver to drop off vehicle(s) or pick up multiple vehicles

Organize travel route and ensure vehicle paperwork is accurate and timely

May need to communicate via 2-way radio or cellular phone

May be asked to clean vehicle and/or keep bus clean during shift
