Clinical Transformation Director, Canada – Full Time Position
Role Description:
Divurgent is now hiring a full-time Director of Clinical Transformation, Canada to lead our Canadian clinical transformation program and for exciting projects throughout Canada and the United States. Must be willing to work up to 50% onsite for customer projects, as assigned. Travel per diem will be offered, where overnight travel is required. Must be able to start mid- to end-September. All Divurgent employees must be COVID-19 vaccinated and be able to successfully pass a background check. Must have a valid driver license. No third-party inquiries or referrals.
The Director of Clinical Transformation, Canada provides leadership and oversight to the development of ongoing clinical service delivery programs, operational work plans, transformation plans, organizational design, and clinical planning.
Responsibilities:
Canadian Clinical Transformation Program Responsibilities
- Apply learnings from previous EHR system upgrades, go-live implementations, and transformation projects where the candidate served as a lead in developing and implementing changes to clinical systems and workflows, integration of other clinical systems (DI, Lab, Pharmacy, etc.), development of standard order-sets, implementation of CPOE, medication bar coding, optimal bed and patient flow, etc.
- Monitor performance of all clinical transformation initiatives and track key performance indicators
- Lead a team of Clinical Transformation Advisors engaged in hospital and health system clinical transformation projects, ensuring an industry leading patient care and clinician experience is delivered by all team members
- Ensure tight integration with project and portfolio management teams, clinical change management, configuration, and user training teams to achieve desired outcomes
- Ensure Canadian healthcare standards and provincial legislation / regulations are incorporated into our projects and CT methodologies
- Monitor all key performance indicators including financial, employee, customer engagement, project health and safety
- Build Canadian Clinical Transformation programs, methodologies, and knowledge libraries
- Ensure that reporting and deliverable deadlines for all clinical transformation programs are met and identify and address barriers to meeting project timelines
Project Engagement Responsibilities
- Host and lead clinical transformation workshops to develop collaborative standards and other key clinical initiatives
- Ensure that clinical programs support the spirit and intent of the health system and comply with all clinical regulations associated with programs and all other contracts
- Lead clinical transformation initiatives at contracted hospitals and healthcare providers
- Alongside customers, build new or revise existing clinical service delivery plans and operational workflows
- Collaborate with appropriate members of the management teams to understand available utilization data, and design and implement programs to favorably impact utilization
- Work closely with physician practice administrators, nursing and clinical practice managers, practice staff and other leaders to create and implement new and revised clinical programs
- Collaborate with care management, data analytics and the management team through a process of continuous quality improvement to identify and address the clinical needs of high-risk patients
Qualifications:
- Experience implementing a variety of improvement methodologies including development and management of performance dashboards with cascading metrics and aligned action plans
- Demonstrated leadership ability, communications, and interpersonal skills necessary to work effectively with physicians, management, staff, external agencies, and patients/families
- Management skills necessary to oversee operations, budgets, and staffing supervision
- Demonstrated strong operational and project leadership skills including developing and implementing systems to hardwire processes, drive accountability, delegate and communicate performance expectations and apply prioritization and proactive problem-solving to ensure achievement of deliverables and outcomes
- Knowledge of evidence-based Clinical Practice Guidelines
- Past EHR go-live experience across either Epic, Cerner, Allscripts and/or MEDITECH required
- Knowledge of how to use Electronic Health Records and computerized statistical methods in clinical quality improvement context
- Ten (10) years of experience managing care in a hospital or a clinically integrated region with at least five (5) years of experience leading healthcare operations and/or systems redesign
- Demonstrated knowledge and experience applying quality, safety and clinical integration best practice approaches to systems redesign and care transformation
- Ability to communicate Clinical Quality and Disease Management concepts, methods and tools in a clear and persuasive manner in verbal and written forms to a variety of audiences
- Masters or Bachelors degree in healthcare administration, nursing, or related field. CHE preferred
