Job Description

LifestyleRx is a precision lifestyle medical practice, combining medical expertise with a team-based approach including one-to-one and group medical visits, nutritional coaching, education, and digital tracking. We are currently looking for a Patient Success Specialist to join our team.


Responsibilities will include:

  • Assist moderating group medical visits, which includes, but not limited to, monitoring group chats, present weekly video content, support patients with Zoom connectivity issues, and etc.
  • Other ad hoc tasks as required.
  • Support the LifestyleRx practitioners (physicians and Registered Dietitians) with patient enquiries, appointment cancellations and/or no shows, and etc.
  • Introduce and onboard patients into the LifestyleRx Type 2 Diabetes Reversal Program.
  • Follow up with clinics and/or labs to help resolve communication/fax issues, missing results, or referrals.
  • Respond to patient inquiries on a timely basis, ensuring they have access to the right services and/or resources, including, when necessary, recommending additional services that may support their journey to remission.
  • Provide dedicated support to new patients during the onboarding process, ensuring a seamless transition and positive experience.
  • Foster and maintain strong relationships that establishes LifestyleRx as the trusted service provider for all things required for their reversal journey.

Qualifications include:

  • Computer proficiency and an ability to learn new programs.
  • Prior experience in healthcare and/or a similar role would be an asset but not required.
