Job Description

JOB DESCRIPTION
JOB TITLE: Behavioural Supports Lead
DIVISION: Resident Care Division
DEPARTMENT: Operations
Equivalent Full-time
LOCATION: Tuxedo Villa
DATE: May 29/23
Reporting to the Director of Care of the Home, the role of the Behavioural Supports Lead (BSL) 1.0 EFT will focus on improving the lives of people who have mental health, dementia or other neurological conditions so they may experience the highest quality of life.
Overall, the BSL is accountable for promoting and contributing to the development of a person-centred philosophy of care for people living with responsive behaviours recognizing the behaviours are often intended to communicate needs or concerns.
Through partnership with people living in the home, families, team members, Extendicare head office teams and external stakeholders (e.g. hospitals), the BSL will support the delivery of relationship-based care. In particular, the BSL will collaborate with the people living in the home, families and care team members to assess, develop and implement plans to help meet unmet needs of those living with responsive behaviours as well as build capacity among team members to have the skills and tools to support relationship-based care.
Accountabilities
  • Facilitate weekly rounds with the interprofessional team to:
  • Develop care plans in collaboration with the person living with responsive behaviours, families, the interprofessional care team, specialized regional consultation teams (e.g., Geriatric Mental Health Assessment Team), and the hospital (if appliable).
  • Work flexible hours to meet the care needs of the people living with responsive behaviours and the operational needs of the home, as required.
  • Ensures that there is a current care plan implemented consistently with goals that focus on optimizing the person’s quality of life and function in alignment with their and their family’s wishes; and
  • Support transitions from hospital to home and vice versa through partnership and open communication/information sharing among all stakeholders.
  • Promote approaches that are person-centred, age appropriate, and honour the person’s dignity, culture, and wishes.
  • Promoting and preserving the person’s abilities, independence, and self-esteem;
  • Role models effective communication and interpersonal skills when engaging with people who live in the home, family members, team members and internal and external partners.
  • Promoting the person’s inclusion in their environment and adapting the setting to meet their needs; and
  • Provide education and training to staff and leaders related to supporting people living with responsive behaviours (e.g. person-centred language; relationship-based care; Gentle Persuasive Approaches (GPA), PIECES, etc.).
  • Participate in and co-lead quality improvement activities that support continuous quality improvement and collaboration.
  • Collaborate with the Social Worker in the home to provide education to families to support them with the effects of caregiving.
  • Demonstrate knowledge in dementia care, delirium, mental health conditions and the impact it has on a person’s quality of life, caregivers, and care partners.
  • Assess learning needs and serve as a knowledge-to-practice facilitator to enhance the team’s ability to support those living with responsive behaviours (e.g., design and role modelling of approaches outlined in care plans)
  • Participate in assessment of the person’s needs based on establishing a trusting relationship and the use of evidence-informed tools and resources.
  • Collaborate with the recreation and programs team members to promote non-pharmacological approaches for respectfully engaging and supporting people living with responsive behaviours.
  • Deliver person-centred care including but not limited to:
  • Contributing to the development of the person-centred philosophy of care
  • Identify and implement mitigations to risks related to the person’s unmet needs.
  • Developing strategies to promote optimal quality of life for the person.
Qualifications
  • Understands and upholds all the regulatory requirements under applicable legislation in the respective province/jurisdiction as well as Extendicare policies and procedures
  • Ability to coach, mentor and provide feedback to team members caring for people with responsive behaviours while fostering a positive work culture
  • Registration or eligible for registration with the applicable nursing association in the respective province (e.g. Associated of Regulated Nurses of Manitoba, etc.)
  • Certified trainer in Gentle Persuasive Behaviours (GPA) and PIECES
  • A well-developed clinical understanding of dementia (i.e., amnesia, agnosia, aphasia, altered perception, etc.) and the ability to appreciate its unique impact on the person and their caregivers
  • Extensive clinical knowledge and skills in caring for seniors (i.e., understanding diagnoses, signs and symptoms; and treatment)
  • Knowledge of various models of care for people living with responsive behaviours (e.g., Butterfly, Montessori, etc.)
  • Minimum three (3) years direct clinical care experience in long-term care and/or seniors’ care, preferably with people living with responsive behaviours
  • Licensed Practice Nurse or Registered Nurse in good standing with the Manitoba College of Nurses
  • Working knowledge and experience in non-judgmentally documenting in an electronic health record (e.g., PointClickCare)
