We are looking for a Clinical Practice Specialist- Learning and Development to provide one on one care in the community/home setting as part of our interdisciplinary care team.

Advances Standards of Care and Professional Practice
  • Collaborates with interdisciplinary patient care teams, operations and quality and professional practice leadership supporting the implementation of local, regional and provincial strategies that will advance excellence in Community Support outcomes from a population health lens.
  • Continually updates personal knowledge regarding current best practices regarding PSW services, trends in community care, College of Nurses of Ontario (CNO) standards and guidelines, and adult education methods/strategies
  • Inspires Personal Support Managers to seek out knowledge independently and to continually improve in the performance of their role
  • Develops and implements standardized orientation for newly hired PSMs, and CSWs, including
  • Develops and implements patient assessment and care documentation system for CSWs and Personal Support Managers in accordance with CNO standards
  • Training local branches in the delivery of CSW orientation
  • Continually updating and circulating revised CSW orientation in response to branch feedback and organizational/industry changes
  • Supports the IPAC program in the absence of the manager of IPAC
  • Evaluating delivery of CSW orientation at local branches and addressing deviations from standards.
  • Supports CarePartners’ strategic priorities relevant to professional practice and ensures leadership is informed of emerging trends in technology, health care, and adult education for CSW's and Personal Support Managers
  • Works closely with Directors, QPP team, Nurse Managers, and Personal Support Managers to identify common educational needs, develop common solutions, implement educational sessions and circulate resources both for nurses/CSWs education and patient/family education.
  • Promotes and maintains the application of standards of care, best practice and/or core competencies into the development of processes and policies supporting CSW practice in home and community care
  • Champions a culture that is committed to and passionate about patient-centered, high-quality and integrated care and the identification of transformative opportunities in enabling point-of-care excellence in every patient and caregiver interaction


Practice Analyst and Advice


  • Collaborates with the local CarePartners branches to identify issues and trends regarding practice gaps and develops proposal, in collaboration with leadership, to mitigate activities and ensure alignment with high-quality patient-centred practice and programming expectations
  • Provide strategic advice on CSW practice and programming issues and themes


Build Capability and Education
  • As part of the program of Personal Support Manager orientation, provides orientation to each newly hired Personal Support Manager in
  • CarePartners promotes and facilitates, as appropriate, the immunization of employees in order to provide a safe working environment for its employees, to deliver safe care to patients and to fulfill its commitment to evidence-based health promotion. In accordance with Public Health guidelines CarePartners has implemented safety protocols to prevent the spread of infectious diseases such as COVID-19. All employees will be required to wear appropriate Personal Protective Equipment as directed by CarePartners and mandated by public health authorities. All new hires of CarePartners are required at this time to demonstrate that they have been fully vaccinated against COVID-19, in accordance with our policies and procedures
  • Able to communicate in both oral and written form and fluent in English
  • Supports local site-specific training and coaching initiatives that are informed by key performance indicators, such as patient experience, outcomes measures and risk events.
  • A valid driver’s license and vehicle
  • Certification in Dementia Care or willingness to obtain
  • Graduation from an approved Registered Nursing Program or Registered Practical Nursing Program
  • Dementia Care
  • Strong organizational and planning skills
  • Provide consultancy service and support to leadership and frontline staff for the promotion of practice and programming excellence, in line with CarePartners internal and regulatory standards
  • Works closely with Patient Care and Operations leadership and collaborates on clinical orientation/role developments
  • Ability to effectively teach individuals and groups
  • Special Functions (face to face or virtually)
  • Formal education or experience in adult education
  • Certification in Gentle Persuasion Approach or willingness to obtain
  • Participates as a member of a variety of internal and external committees related to role and expertise
  • Clinical competencies consistent with care provided by CSWs including catheterization, G-tube feeds, palliative care, etc.
  • Assists in the development of communication with evidence-based clinical and research content, e.g. PPRE emails, company newsletters
  • Leadership skills, knowledge of change management
  • Documentation of patient assessment and care
  • Registered with the College of Nurses of Ontario and able to practice nursing in Ontario
  • Supports the consistency of CSW care practices across the branches
  • Computer skills including Microsoft Office Suite
  • Champions the use of digital learning platforms and identifies infrastructure necessary to enable nimble and responsive learning for frontline team members
