Hours of Work: Every Monday & Tuesday, 8 hrs; as per schedule


Summary Of Duties


The Weekend Team at Sunnybrook consists of a Social Worker, Occupational Therapist, and Physiotherapist. Social Workers from various acute care units (General Internal Medicine, Oncology, Cardiology, Surgical Oncology, Burns and Trauma) rotate into providing weekend coverage. The Weekday Backfill Social Worker provides 2 day backfill coverage for the Social Worker and the acute care unit that provided weekend coverage. The Weekday Backfill Social worker will use a person centred model of care that enables them to optimize their scope of practice, improving effectiveness and efficiency of care, and the patient experience. The Weekday Backfill Social Worker will collaborate with the acute care unit interprofessional team to provide patient follow up, assess new patients, complete referrals/applications to facilitate discharges or transitions in care.


The candidate will be able to work in a fast paced environment determining priorities according to the patient’s goals and estimated discharge date. The successful candidate will practice in accordance with the appropriate legislation and organizational policies for decision making, care planning and coordination of care.


  • Collaborate with Acute Care Unit teams, CCAC, other community resources to facilitate discharges
  • Problem solve to facilitate weekday discharges
  • Collaborate with colleagues regarding weekend handover list outlining pending issues for focused weekday follow-up
  • Update ALC status in Sunnycare
  • Assess and confirm medical stability to determine if patient is requiring Alternative Level of Care
  • Complete assessments and applications for weekday discharge and transition planning


Qualifications/Skills


  • Minimum 2 years of experience working as an acute care hospital required
  • Demonstrated understanding of caregiver stress or family dynamic factors that may impact discharge planning
  • Master’s Degree in Social Work required
  • Registered and a Member in good standing of the OCSWSSW
  • Knowledge and skills in applying relevant theoretical frameworks in particular systems theory, crisis intervention, brief solution focused perspectives and adjustment to illness/injury
  • Emergency Department experience an asset
  • Conducting Psychosocial assessments with a priority on identifying complexities regarding discharge over the weekend or Monday
  • Competency in adhering to the SW Standards of Practice at SHSC
  • Advanced negotiation and mediation skills to balance patient, family/caregiver, team, organizational and access to care needs and opportunities.
  • Knowledge of Social Work Competencies in Hospice and Palliative Care
