To learn more visit www.clevelandclinic.ca.*GENERAL SUMMARY:*Cleveland Clinic Canada is seeking experienced PSWs to provide 1 on 1 home care to patients in the community. The successful applicants will provide an outstanding patient experience through a caring and compassionate approach. With a focus on meaningful companionship, they will assist patients in carrying out their activities of daily living as outlined by their personal support plan. In many cases, patients will require full-time support, thus applicants must be willing to work non-traditional hours, including evenings, weekends and holidays.*Major Roles/Responsibilities:** Bathing and Dressing* Oral and Hair Care* Medication Reminders* Companionship* Accompaniment (Medical Appointment etc.)* Transfers (i.e. getting in/out of bed, getting in/out of cars etc.)* Bedrest, Standing and Ambulation Assistance* Light Housekeeping* Light Meal Preparation* Laundry / Linen Change*MINIMUM QUALIFICATIONS:**Education, Knowledge, Skills and Abilities** Current PSW certificate from a recognized institution* Caring and compassionate* Experience in homecare is an asset* Experience working with patients in residential settings is an asset* Excellent communication and interpersonal skills* Maintain effective communication and good relationship with residents, families, Power of Attorney (POA)* Eligible candidates must be able to work non-traditional hours including early mornings, evenings, weekends and holidays if required* Current Basic Cardiac Life Support (BCLS) is mandatory* Valid Level C CPR and first aid certification* All applicants must successfully pass the prescribed Vulnerable Sector Check* A valid driver’s license and vehicle is an asset*CCHS ORGANIZATION-WIDE COMPETENCY ASSESSMENT REQUIREMENTS:*All employees are expected to meet the standards of performance outlined in the Organization-Wide Competencies listed below as applied to the position.*Customer Service Orientation *includes attitude, behaviour, interpersonal skill, and problem solving that enable an employee to respond to internal and external customer needs and expectations in a positive manner.*Adaptability *includes teamwork, flexibility needed to fulfill job responsibilities, adapting to changes in work environment and accepting supervisory feedback.*Efficiency and Effectiveness *includes quantity and quality of desired work, as well as organization skills necessary to perform successfully.*Essential Job Requirements *includes adherence to all relevant policies, procedures, and guidelines affecting the work environment, as well as maintenance of required competencies and communication skills.Job Type: Full-timeSchedule:* Day shift* Evening shift* Morning shift* Night shiftLicence/Certification:* PSW certificate from a recognized institution (required)* First Aid & CPR Certification (preferred)Work Location: On the road
