Program Overview – First Step To Home, Community Care Unit


First Step to Home is a transitional program with housing as an enabler for men 55 years and up who have been street involved and experiencing mental health and/or addictions issues. The program provides onsite psychosocial primary care supports and services including: case management; harm reduction and addiction supports; supportive counselling; 24-hour staffing; assistance with activities of daily living; life skills development; social and recreational activities; volunteer opportunities; and follow up support.


Support services at 841 Queen E Streets to Homes & 1117 Gerrard Street include case management, harm reduction, addiction supports, supportive counselling, social and recreational activities, and low barrier programming.


What You Will Do


  • Provide daily oversight and supervision of program staff at all locations.
  • Provide program and job orientation, training plans for new staff members, complete performance appraisals, provide coaching, training, skill development and follow up as needed.
  • Support the Manager in building and maintaining internal and external partnerships and collaborative activities.
  • Support Manager in the development and implementation of program initiatives, such as food access, peer involvement, and social/recreational and life skills programming.
  • Support the Manager with the implementation of an effective evaluation strategy, program/operational protocols, policies/procedures being carried out within all programs, and data tracking systems, reporting tools, and completion of reports.
  • Support the Manager with the planning and implementation of staff development.
  • Support with implementing the operational responsibilities of programs/services/initiatives.
  • Support Manager in the development of strategies to address tenancy and building issues.
  • Support Manager with recruitment processes for new staff
  • Support & guide case management staff with the development and execution of Coordinated Care and Case Plans for clients.
  • Provide support to program staff regarding program issues, team building, and crises incidents through individual meetings with staff and participation in team meetings.
  • Guide program staff to support residents who are facing tenancy issues at, 1117 Gerrard Street, 650 & 841 Queen St E, including pest control, hoarding, arrears, inspections, notices and work orders.
  • Support the effective utilization, training, and support of volunteers and students throughout the various programming activities.
  • Prepare statistical and program reports.
  • Ensure that the equity objectives and community development policies of the organization are followed within the programs.


What You Bring To The Team


  • 3+ years of experience in implementing programs; working with homeless and vulnerably housed individuals; experience with older adults and seniors; supervising staff, preferably in a unionized environment; knowledge of RTA and occupancy agreements, and property management.
  • Bachelor’s Degree in Social Work or a related discipline.
  • 2+ years’ experience supervising staff.

  • Excellent interpersonal and communication skills, which promote effective working relationships amongst program staff, external agencies, program participants, and tenants.
  • Strong computer proficiency.
  • Excellent writing and editing skills.
  • Exercise excellent judgment, flexibility, creativity, and sensitivity to changing situations/needs.
