POSITION SUMMARY:
The Physician Assistant (PA) will practice within his/her full scope of practice. The PA will work with a physician, and other Allied Health Professionals in a rural setting practice
ips: Provide a summary of the role, what success in the position looks like, and how this role fits into the organization overall.
Responsibilities
Including, but not limited to:
·Complete health assessments including health history and physical examinations
·Provide wellness care and health screening
·Diagnose and treat minor illnesses and injuries
·Screen for and treat chronic illness and monitor stable chronic illnesses
·Care of episodic illness
·Determine the need for, and order and interpret reports of X-Rays, ECG’s and diagnostic ultrasound.
·Monitor the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and
counseling interventions.
·Determine the need for, and the implementation of health promotion and prevention strategies for families and
individuals
·Formulate and communicate medical diagnosis or disorder.
·Consult with a physician in accordance with the standards for physician consultation.
·Prescribe drugs Collaborate with the team to support client care and needs
·Gathering and documenting statistical information as required
·Implement, evaluate and refine individualized care plans for the client and family care- giver based on best
practice guidelines.
·Facilitate coordinated access to services in areas such as assistance with daily living, crisis intervention,
treatment, health promotion and prevention.
·Facilitate linkages and referrals to appropriate services, supports, and resources share expertise with Clinical
Team
·Document in EMR
·Arrange case consultation and case conferences as needed
·Practice based on sound theoretical knowledge, evidence and best practice guidelines

Team Development:
·Promotes awareness of the RSLFHT services and programs
·Actively participates in staff, team and committee meetings as appropriate
·Provides leadership and mentorship to students on placement at RSLFHT.
·Uses community assessment data to identify community needs and resources and develops implements and evaluates appropriate programming.
Communication:
·Communicates effectively with health care team members to create a cohesive team and seamless services to
the community
·Communicates effectively with all clients, families, peers, other health care professionals and community
partners
·Participates in interdisciplinary meetings as required
·Represents the organization to external stakeholders as required.
Professional Development:
·Maintain and develops professional competence
·Participate in self-directed learning to ensure that practice remains relevant
·Participate on inter-professional committees to promote professional/interpersonal development.
·Participate in clinical projects, program review and development as required.

·Facilitates appropriate in-house and external education sessions

·Performs other associated duties as required.
Qualifications
- Experience in community health in a primary care and or rural setting preferred
- Registered with CAPA
- Graduate of an accredited Physician Assistant Education Program
·Three to five years of experience: preferable in a community setting/Senior/Care
·Proficiencies in assessment,
·Up to date knowledge of community resources and experience in developing community linkages and
relationships
·Understanding of the social determinants of health and evidence-based healthcare practices
·Excellence in problem-solving and conflict resolution
·Experience in program development, implementation, monitoring and evaluation
·Desire and ability to update knowledge and skills through various means including technology-based
opportunities, courses, workshops and conferences.
·Demonstrated ability to work with a team, work independently and work within a community practice setting
·Experience with PS Suites or other Electronic Medical Record
·Experience with Software programs including Microsoft Word, Excel, Outlook and PowerPoint
·Current Ontario Driver’s License
