Program Overview – Adult Developmental Services, Community Care


Funded by the Ministry of Community and Social Services, Developmental Services includes a full range of services to adults who are socially isolated and live with developmental disabilities. The philosophy of this work is in keeping with WoodGreen’s commitment to community development, anti-discrimination, and promoting wellness and self-sufficiency.


What You Will Do


  • Participate in supervision sessions and annual performance appraisals with program manager.
  • Maintain appropriate records and documentation in a timely manner and collect statistical data required for the accountability of the program.
  • Provide crisis intervention and practical assistance to clients as required in homes and community settings.
  • Ensure excellent communication with staff who work on-site at the various specified locations, including Madison Support staff, PSWs, Supervisors and other partners in the community.
  • Respond effectively and professionally to clients, situations and emergencies.
  • Provide comprehensive case management on a person directed basis for individuals with a dual diagnosis or who are living with symptoms of a mental illness and/ or addictions.
  • Network with other relevant agencies, community groups and/or coalitions regarding issues and needs of clients.
  • Provide mentorship, training and/or supervision to student placements and volunteers.
  • Participate in ongoing training and education.
  • Keep current about the range of services available in other agencies.
  • Advocate on an individual basis or with organized groups regarding issues related to the needs of Madison Community Services clients who identify as having a Dual Diagnosis or are experiencing symptoms of a mental illness and/or addictions.
  • Provide daily intensive support for Madison Community Services clients in working towards their goals identified in their CCP.
  • Participate with the Program Manager in case management review, program evaluation and annual operational and staff professional development.
  • Provide crisis intervention as required.
  • Provide Drop-In support to Madison clients on a weekly basis.
  • Participate in the planning and implementation of program social activities.
  • Assess individual needs, assist in setting goals, and create Coordinated Care Plans unique to the individual.
  • Develop trust and rapport with clients receiving services through Madison Community Services in order to identify individuals who could benefit from dual diagnosis support and coordinated care coordination.
  • Promote the healthy functioning and participate in team and unit meetings regularly.
  • Participate in case consultations and or team meetings as requested/required.
  • Ensure the organization’s policies and procedures are carried out.


What You Bring to the Team


  • Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption).
  • Demonstrated experience facilitating psycho-social groups and supporting individuals with significant mental health and/or addictions issues, and/or Developmental Disabilities from a harm reduction, trauma informed, and mental health recovery approach.
  • Masters of Social Work or Bachelor’s Degree in relevant field.
  • 2+ years of experience in mental health, addictions, and/or social work.

  • Ability to support individuals who have complex mental health and/or addiction issues, developmental disabilities, history of trauma, violence/abuse, etc.
  • Excellent organizational and time management skills.
  • Knowledge of theories of addiction, evidence-based treatment models and best practice approaches.
  • Strong de-escalation and crisis intervention skills
  • Comprehensive understanding of the mental health, addictions, developmental services, health sector, and community resources.
  • Ability to work from a client-centred approach.
  • Applied computer skills (MS word, Excel, Internet) and ability to use client information systems.
  • Proficiency in second language, written and oral, is a strong asset.
  • Ability to exercise good judgment and flexibility.
  • Knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinant of health.
  • Self-directed and independent, but also proven experience in working effectively as a team member in collaboration with other community professionals.
  • Strong interpersonal and conflict resolution skills to work collaboratively with all service providers and stakeholders.
  • Excellent oral and written communication skills and documentation skills.
