Overview Case Management Specialist II is a professional who provides case management services to individuals and families in need of assistance. They are responsible for assessing the needs of their clients, developing and implementing plans to meet those needs, and providing ongoing support and guidance. They may also provide referrals to other services and resources, and coordinate with other professionals to ensure the best possible outcomes for their clients. Detailed Job Description Case Management Specialist II is responsible for providing case management services to individuals and families in need of assistance. They will assess the needs of their clients, develop and implement plans to meet those needs, and provide ongoing support and guidance. They will also provide referrals to other services and resources, and coordinate with other professionals to ensure the best possible outcomes for their clients. They will also be responsible for maintaining accurate records and reports, and providing regular updates to their supervisor. Job Skills Required• Excellent communication and interpersonal skills

Job Qualifications
• Bachelor’s degree in social work, psychology, or a related field
• At least two years of experience in case management
• Certification in case management or a related field
• Knowledge of relevant laws, regulations, and policies
• Ability to pass a background check

• Knowledge of assessment and evaluation techniques
• Knowledge of case management principles and practices
• Knowledge of relevant computer software
Job Experience
• At least two years of experience in case management

• Experience coordinating with other professionals
• Experience maintaining accurate records and reports
