Classification: Case Management Clinician


Full-Time, Contract for up to 12 Months


Multi-Site Accountability; Oxford Catchment Area

As a Case Management Clinician you would work one on one or in groups with individuals who choose our program to meet their needs. You will support individuals as they improve their health and quality of life in the face of Mental Health and/or Addictions concerns and the accompanying barriers they experience. The Case Management Clinician assesses the person’s needs, strengths, and supports to co-develop goals and a customized recovery and support plan. You would work from a trauma informed perspective as part of a team and part of a community to advocate, challenge stigma, and prevent harm.

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.


WHAT WE'RE OFFERING:


  • PSLRTA Notice - This posting characterizes this position as currently unionized under the Oxford Collective Agreement. However, please be aware that the organization on September 29, 2022, a vote was held as a result of an application by the Organization under the Public Sector Labour Relations Transition Act. The result was that employees voted to award OPSEU bargaining rights for all employees. Until such time as a new collective agreement is finalized, current terms and conditions of employment and existing, applicable collective agreements, if any, will remain in force and effect.
  • What you'll be making - $32.18 per hour or as per CBA
  • What your benefits will be - you will receive 9% in lieu of health benefits
  • Your pension plan - Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP)
  • When you'll work - 70 hours over a two week period, flexible time schedule


HOW YOU WILL BE MAKING A DIFFERENCE:


  • Ongoing professional skill development/continuing education towards the delivery of mental health services.
  • Monitor symptomology, medication, and activities of daily living to help improve individuals’ ability to function on a day to day basis and within the community
  • Assess clients’ mental, physical and emotional functional abilities and needs and investigates client’s present resources
  • Provide crisis support and crisis prevention planning
  • Maintain current and accurate client documentation records as per agency policy
  • Provide assistance with financial management.
  • Establish a working and supportive relationship with clients
  • Work in collaboration with partners and various CMHA teams including Housing, Court, Crisis, and Concurrent Teams to provide timely interventions
  • Support and assists family members with issues related to the client’s mental illness
  • Liaise with other professionals and with the community for the benefit of the client while adhering to the principle of confidentiality.
  • Provides skill teaching, concrete counselling, and support to clients to assist in goal achievement
  • Advocate for client rights as necessary or ensure self-advocacy
  • Provide support to client’s on other workers’ caseloads as needed
  • Ensure that all program/caseload information is handled according to organizational standards of privacy and confidentiality
  • Provide vocational and occupational support
  • Provide discharge planning
  • Devise a service plan with the client, establishing goals to meet determined needs


WHAT YOU'LL NEED TO APPLY:


Police Vulnerable Sector Check


Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.


We require the following qualifications:


  • Satisfactory Criminal Records Check and Vulnerable Sectors Screening
  • Extensive knowledge of the support/treatment services network in Oxford County and of regional services commonly accessed by the Program
  • Computers skills
  • Valid driver’s license, access to a reliable vehicle, and vehicle insurance in compliance with CMHA Policy
  • Crisis Management/Intervention Skills
  • Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (as an RSW), the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of Ontario
  • Experience and detailed knowledge of serious mental illnesses usually gained by several years of working with this population
  • Demonstrated ability to attend work regularly
  • Ability to advocate and coordinate service access
  • RPN, RN, Bachelor's or Master's Degree in Occupational Therapy, Social Work, Psychology or related field is required (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Extensive knowledge of Services in Oxford County
  • Knowledge of Psycho-social Rehabilitation
  • Ability to build relationships
  • Excellent communication skills
  • Planning and organizational abilities
  • Experience in working in a community-based setting with an extensive knowledge of social assistance programs and both generic and specialized local social support networks is essential
  • Strong active listening and verbal communication skills
  • Ability to develop short and long term treatment plans in conjunction with the individual


We prefer the following qualifications:


  • Bilingual both official languages, preferred; préférence sera accordée aux candidats qui sont compétents dans les deux langues officielles du Canada
  • Knowledge of legal system
  • Experience in crisis intervention/management and crisis resolution
  • ASIST certificate
  • Experience in the treatment of Addictions
  • Current CPR and First Aid certification
  • Extensive knowledge of pharmaceuticals used in the treatment of mental illness & experience in monitoring/administration of medication
  • Experience working with individuals with an ABI or Development Disability
