Social Worker - Regular Part-Time - C6 (2023-4909)

Summary Of Duties

To provide a full range of social work services to General Internal Medicine (GIM) gyne/oncology, and plastic surgery patients and their families on an acute care unit. Services include psychosocial assessments, counselling, discharge planning, linkage to community resources, facilitating Advance Care Planning Conversations and addressing complex care issues.

The successful candidate will participate as a key member of the patient care team on General Internal Medicine.

Qualifications/Skills

• Minimum two years of experience working as an MSW in a Hospital setting; experience working with GIM, oncology and surgical patients preferred
• Member in good standing of the Ontario College of SW and SSW
• Master’s Degree in Social Work required
• Excellent communication, organization skills and time management skills
• Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education
• Strong ability to coordinate community resources related to pain and wound management and palliative care support.
• Commitment to student education
• Excellent assessment, interviewing and problem solving skills
• Superior ability to manage multiple tasks and set priorities in a fast-paced environment
• Strong teambuilding skills and conflict resolution skills
• Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care

• Knowledge of the Health Care Legislation, health care system and community resources
