About The Opportunity
Please Note: This is a designated position. The successful candidate must self-identify as African Nova Scotian in order to be considered for this role.
Reporting to the Primary Health Care Manager (PHC), the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios specifically related to the health and wellness of Women of African Descent. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in the Nova Scotia Sisterhood(NSS) portfolio.
The scope of practice for the PHC Coordinator will include collaborative and work with African Nova Scotian leaders and community. Liaise with health care providers to support change management strategies that are inherent in the services plan for primary health care. The PHC Coordinator has a responsibility for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the leadership team. The PHC Coordinator is responsible for supporting the ongoing development and implementation of the Nova Scotia Sisterhood quality program. The Primary Health Care Coordinator activities are guided by the Primary Health Care priority areas, multiple stakeholders and by community needs therefore, requires a professional approach. This work requires recognition and application of best practices and application of evident informed decision making. The Primary Health Care Coordinator serves as a resource to other team members in Primary Health Care and the Department of Family Practice as well as within Nova Scotia Health as required.
About You
  • Proficiency in cultural competencies
  • Experience working with African Nova Scotian community considered an asset
  • Demonstrated leadership in facilitation and multiple stakeholders and service providers
  • Proven ability to build, enhance, foster and maintain effective relationships
  • Ability to think critically, quickly and accurately, creative problem-solving ability
  • Ability to analyze information to identify priorities and emerging issues
  • Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management
  • Ability to effectively organize heavy workload with multiple competing priorities
  • Superior communication skills – written, oral, presentation
  • Baccalaureate degree preferably from a health discipline/health related science
  • Organizational and process management skills
  • Coordination and Project Management Skills
  • Must have a high level of judgment in order to make independent decisions related to responsibilities
  • Competencies in other languages an asset, French preferred.
  • Ability to function independently as well as a team member
  • Superior time management skills in order to prioritize and complete assignments
  • Experience working in a family practice setting with family physicians an asset
  • Sound knowledge and skill working in an interdisciplinary setting
  • Valid Nova Scotia Driver’s License
  • Current registration with professional association if applicable
  • Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
  • Proficiency in Microsoft Office
  • Two years' of recent related experience
  • Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset

Compensation and Incentives
$30.30 - $36.79 hourly
