About The Opportunity
Reporting to the Primary Health Care Manager, the Primary Health Care Coordinator (EMR Support) will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator (EMR Support) role will be focused on supporting EMR reporting/optimization/efficiency and have responsibility to lead the planning, ongoing development, implementation and evaluation of services and project initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders across the Eastern Zone to ensure zonal support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies.
The incumbent will support EMR reporting/optimization/efficiency for collaborative family practice teams in primary health care.
The Primary Health Care Coordinator (EMR Support) will work collaboratively with family physicians and other health care providers and teams across the Eastern Zone to support quality improvement and change management strategies that are inherent in the services plan for primary health care.
About You
We would love to hear from you if you have the following:
  • Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
  • Valid Nova Scotia Driver’s License
  • Efficient in PHC Electronic Medical Records (Telus Med Access and/or QHR Accuro), Microsoft Word, Excel, PowerPoint, Outlook – smartsheets an asset
  • Competencies in other languages an asset, French preferred.
  • Sound knowledge and skill working in an interdisciplinary setting
  • Understanding of Behavior Change Techniques
  • Broad knowledge of the health system with high level understanding of population health approaches, community development, and primary health care.
  • Experience working in a family practice setting with family physicians an asset
  • Baccalaureate degree preferably from a health discipline/health related science, consideration maybe given to applicants with equivalent combination of relevant education and experience
  • Organizational and project management skills. Superior time management skills in order to prioritize and complete assignments
  • Experience in, change management strategies, and quality improvement is an asset
  • Current registration with professional association if applicable
  • Two years' of recent related experience
  • Ability to analyze information to identify priorities and emerging issues
  • Superior communication skills – written, oral, presentation
  • Coordination and Project Management Skills

Hours
  • Permanent Full-time Position; 70 hours bi-weekly
Compensation and Incentives
$30.30 hourly - $36.79 hourly
