About The Opportunity
Reporting to the Primary Health Care Manager, the Primary Health Care (PHC) Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator role will be focused on supporting, reporting, optimization, efficiency and have responsibility to lead the planning, ongoing development, implementation and evaluation of services and project initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders across the Western Zone to ensure zonal support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The PHC Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies.
The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support quality improvement and change management strategies that are inherent in the services plan for primary health care.
About You
We would love to hear from you if you have the following:
  • Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
  • Current registration with professional association is an asset
  • Competencies in other languages an asset, French preferred
  • Organizational and project management skills
  • Efficient in PHC Electronic Medical Records (Telus Med Access and/or QHR Accuro), Microsoft Word, Excel, PowerPoint, Outlook – smartsheets an asset
  • Experience in change management strategies, and quality improvement is an asset
  • Baccalaureate degree preferably from a health discipline/health related science.
  • Experience working in a family practice setting with family physicians an asset
  • Two years' of recent related experience

Hours
  • Six (6) Permanent full-time position; 75 hours bi-weekly.
  • Shifts are flexible depending on needs of the program, evenings and weekends included.
  • Travel to various sites will be required (significant portion of the position)
  • Location will be determined based on the successful candidate within Western Zone.
Compensation and Incentives
$30.30 - $36.78 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
