Spread Your Wings, a large, experienced provider of supportive living services has an immediate need for an enthusiastic Case Manager to join our team in the Santa Rosa area. Candidate must have strong communication skills and works well under a fast-paced environment. Spread Your Wings is a for-profit Supportive Living and Independent Living Services agency, and we also have office locations in many other areas in the northern California/Bay Area communities.
Our mission is to provide high quality Supported Living Services to persons with developmental disabilities with respect, dignity, and pride to guide our consumers and link them to all available resources in the counties we serve.
Candidates must have reliable transportation and preferably reside in/within Santa Rosa area, and be available for occasional after-hours emergencies/calls.

Responsibilities And Duties
- Assessment, planning, implementation, and review of person-centered support plans for people served.
- Ensures quality, health, and safety with client-centered approaches for individuals living in a community setting.
- Comply with all standards to assure the health and safety of all staff and clients we serve.
- Provides supervision, support, and training for Direct Care Staff as they assist individuals served.
- Schedule Direct Care Staff in accordance with ISL budget and services identified on the Individual Support Plan and providing emergency coverage, if needed.
- Ensures compliance with federal, state, and local regulations while expanding opportunities for persons serviced to participate in community life as they choose.
- Accurate and timely completion of ISPs, Monthly Reports/Addendums, client financials, QA walk-through, and other relevant daily/weekly/monthly documentation (logs, fire and tornado drills, client medical information, MARs, etc.)
- Providing on-call direct support and coverage coordination, if needed.
- Conduct weekly house checks for assigned clients on caseload (requires local travel between clients' residences).
- Develop and implement the Individual Support Plan and assist in development of the Individual Supported Living (ISL) budget for each person served.
- Manage and monitor client finances within required time frames.
- Influencing and procuring an ISL budget that addresses all client needs and articulating elements to appropriate staff.
- Responsible for submitting reportable events within required time frames, both internally and to Regional Center Staff.
- Attend meetings/training/workshops, as assigned.
- High School Diploma or equivalent required; Associate's degree or above preferred.
- Must be able to drive/transport to different locations within assigned area.
- Minimum of one year of leadership and/or supervisory experience.
- Must pass Department of Justice (DOJ) background check.
- Requires a California Drivers License, Proof of Vehicle Insurance, and current driving record from Department of Motor Vehicle (DMV).
- Minimum of 2 years of experience providing direct support/care to clients in a Mental Health/ Intellectual Disabilities setting.
- 50% Coverage of Medical/Dental/Vision Benefits
- Hourly Wage (DOE): $22-24/hour.
