CMHA-S/M JOB POSTING - 23-06 Housing Case Manager, Sudbury East
Department: Community Development & Housing
Employment Status: Full-Time, Permanent
Hours per Week: 35 hours per week. May include weekday/weekend/evenings
Work Location: Works independently out of community housing buildings in St. Charles, Warren and Noelville.

Responsibilities
Reporting to the Coordinator, Housing Case Management and working in partnership with the Manitoulin-Sudbury District Service Board (MSDSB), the Housing Case Manager provides supports to individuals residing in community housing units operated by the MSDSB.
Other Duties Include
Build rapport and assist with tenancy-related issues
Provide support to individuals with the development of goal-focused care plans and wellness strategies
Provide direct, hands-on support to individuals in maintaining their living environment and assistance with instrumental activities of daily living skills
Develop educational programs relevant to identified needs
Ensure all file management and reporting functions are maintained in a
comprehensive and timely manner

Qualifications
Completion of a 2 year diploma or degree in a health or social services discipline. Combination of post-secondary education and 5+ years of experience that reflects the job description and requirements of the current position will also be considered. Minimum one year experience in related position.
Professional Designation/Certification ( one of the following required )
  • Working towards obtaining one of the above within 6 months of hire
  • Registration, in good standing, with a regulating professional body (e.g.OCSWSSW); or,
  • Certification, in good standing, with a professional certifying organization within health or social service fields (e.g. CPRRP from PSR Canada); or,
Organization must have a written code of ethics, core values and standards that are consistent with those of CMHA-S/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHA-S/M.
Access to a reliable vehicle required to complete job duties
Proficiency with Microsoft Office software required
Knowledgeable of community agencies and resources
Bilingual requirements will be posted in accordance with the FLS Designation Plan. Bilingualism is required for 40% of positions. If a position is designated bilingual and no bilingual candidates (both internal and external) meet the qualifications, unilingual candidates may be considered.
Salary
Bachelor Degree:
Annually, Step 2: $48,719 - Step 5: $57,316
Hourly, Step 2: $26.77- Step 5: $31.49
College Diploma
Annually, Step 2: $43.943 - Step 5: $51,697
Hourly, Step 2: $24.14 - Step 5: $28.41
