Closing the Gap Healthcare is seeking a full time Care Team Coordinator (CTC). The CTC’s role is an integral contribution to the quality of care provided by CTG. You will be coordinating client referrals with our team of Providers, providing empathetic client service and the administrative duties.

This Care Team Coordinator role is a hybrid position based out of our Toronto and Mississauga office.

The Successful Care Team Coordinator Will Have

• An outstanding commitment to providing excellent client centered customer service
• Precise attention to detail and efficiency in data entry
• Effective verbal and written communication skills
• Previous work experience in healthcare setting considered an asset
• A diploma from Community College in Business Administration, Office Administration or Medical Office Administration and/or Grade 12 Diploma with related experience

The Role Will Require

• Accurately and efficiently assessing team requests and addressing them immediately or escalating as appropriate
• Performing data entry requirements for all clients using the CTG computer system
• Strong problem-solving skills
• Supporting daily office operations and program leadership team
• Working effectively with other team members
• A passion for challenges and working with team members to ensure a great experience
• Effectively triaging calls to ensure effective call management
