Homecare Hub is a innovative provider of affordable, safe, personalized, and small shared care homes for those with caregiving needs such as dementia, disability, injury, fall risk, or chronic illness. The company helps individuals stay out of large institutional nursing facilities and instead age with dignity in their own community. Homecare Hub provides small care homes for 2-12 people, as well as customized care homes on demand. Additionally, those with caregiving needs can even host a shared care home in their own homes to save even further.


Our solution is a needed alternative to nursing homes which are expensive to build and operate, and unable to meet the care needs of the future. We also patients find high quality home care and other services to help seniors and other homebound patients be cared for safely in their own home. There is no other system on the globe like Homecare Hub, and we have aggressive expansion plans to meet up with the demand and to ultimately change our society for the better for those with caregiving needs.


Role Description

We are seeking a compassionate social worker, nurse, or personal support worker who will provide care coordination for families and patients. Ideally this person is based out of the Toronto area and would work in a hybrid environment, but we are not completely closed off to exceptional candidates who do not live in the Toronto area.


The Care Coordinator will be responsible for coordinating care for individuals with caregiving needs, including managing cases, communicating with clients and their families, overseeing homes, creating and procuring new homes, collaborating with partner providers, managing home care services, and providing excellent customer service. The Care Coordinator must work collaboratively with the interdisciplinary team and external partners to ensure that care is being provided at a high standard.


Position Overview

•Assisting patients with service coordination to meet care needs.

•Providing patients and families with guidance and resources reflective of care and situational needs.

•Maintaining accurate records and preparing reports.

•Following up with patients and service providers in order evaluate service needs and resolving issues that may arise.

•Managing relationships with existing healthcare and senior care organizations, and assisting in establishing new partnerships.

•Working with the technology team to assist and coordinate the design, testing and roll-out of product enhancements and new services and features to the Homecare Hub platform.

•Assisting new and current users with navigating the Homecare Hub platform.

•Routinely reviewing new patient leads and referrals, and conducting care coordination in a timely matter.

•Handling calls while delivering exceptional customer service

Qualifications

•Must be a RSW or MSW (RSSW’s will be considered), or be a RN, RPN, or PSW with appropriate experience.

•Minimum of three (3) year related work experience

•Previous experience working in a Health care environment

•Previous experience and knowledge of home care services, including care types and eligibility. Understanding of senior housing an asset as well.

•Care Coordination and Case Management skills

•Working knowledge of computer software applications preferably in a Microsoft Office environment

•Demonstrated commitment to client service and satisfaction

•Excellent interpersonal, listening, communication, organizational, time management and problem-solving skills

•Strong judgment/decision making skills

•Ability to remain calm and composed when dealing with difficult people or situations

•Adheres to confidentiality policy for sensitive patient data and information

•Demonstrated ability to work in a team environment and collaborate with others in assisting with the delivery of services

•Entrepreneurial minded with the ability to adapt to change and creatively solve challenges

•Knowledge of Home and Community Support and other funded home care services preferred

•Access to a vehicle and ability to travel to various locations preferred

•Current member in good standing with the OCSWSSW or ONA

•Strong skills in Home Care, including knowledge of local resources and providers, and in coordination of care services

•Excellent organizational and time management skills, with the ability to prioritize and multitask in a fast-paced environment

•Ability to work both independently and collaboratively as part of an interdisciplinary team
