Previous patient care experience in health care environment or health related community work, preferred

Able to discern relevant information to make effective judgments and decisions to manage care

Demonstrates excellent written and verbal English skills

Completed a recognized program for personal support worker

Current Basic Rescuer Certificate in Basic Cardiac Life support or willingness to obtain within 3 months of hiring

Member of the Ontario Personal Support Worker Association (OPSWA) and/or Personal Support Network of Ontario (PSNO) an asset

Minimum two (2) years related acute hospital administration experience, preferably in a supervisory/managerial capacity

Excellent time management with demonstrated ability to coordinate numerous and diverse activities, organizing and prioritizing appropriately to meet demands and expectations

Minimum three (3) years recent direct patient care experience in an acute medical or hospital setting

Excellent written and verbal communication, interpersonal and negotiation skills

Knowledge of legislation provisions which impact on work performed (i.e. Excellent Care for All Act, Employment Standards Act etc.)

Knowledge and familiarity with staff scheduling, workload measurement, budgets, labour relations and hospital operations preferred

No Dialysis experience is required.

10-15 years professional experience in pharma commercial roles (Marketing/Sales) with proven experience of PSP/Vendor management.

Proven experience in pharma companies (P&L roles) and vendor would be a strong asset

Administers financial means tests and manages all aspects of financial assistance within the program as applicable

Job duties include, but are not limited to the following:

Single point of contact for patients enrolled in the support program and prescribing physician, if applicable

Conducts welcome calls to gather relevant information, introduces patients to the support program, and obtains patient consent as required.

Contact clients periodically to confirm adherence and coordinate delivery of medications (including date, time)

Responds to telephone and paging systems for requests for transportation of patients, related equipment, and supplies.

As directed by the RN/RPN/LPN/NP, takes temperature, pulse, respiration, and weight and obtains non-sterile specimens such as urine and stool.

Transports patients using mechanical aids such as wheelchairs, lifts and stretchers.

Assists in the admission, discharge and transfer of patients including the transfer/care of clothing and personal belongings.

Sets up charts for new patients and attaches documents to charts as directed by the RN/RPN/LPN/NP and according to established protocols.

Attends internal/external committees, patient care meetings and training sessions as assigned.
