As a Rehabilitation Assistant you will provide therapy assistance to the Community Physiotherapist and Occupational Therapists in the provision of PT and OT services to Home & Community Care Support Services HNHB eRehab patients in order to maximize the patient’s function, independence, and safety in their respective environment. Hours of Work Monday to Friday, 830am-430pm **Demonstrated computer proficiency is required – testing to be completed**


What will you do?


  • Promotes maximum independence of the patient in the home, in the community, and at work to support re-integration into the community
  • Completes and maintains documentation regarding patient intervention in accordance with HCCSS HNHB policies and procedures and CPO/COTO guidelines
  • Meets with the registered therapist regularly to discuss patient treatment plan and progress
  • Contributes to a safe and healthy environment by following safe work procedures, reporting injuries, illnesses, and unsafe working conditions
  • Carries out all duties in accordance with HCCSS HNHB policies and procedures and Guidelines for the Use of Supportive Personnel developed by CPO and COTO
  • Works within the basic principles of patient safety by doing the right thing for the right patient, using the right method at the right time
  • Researches assessment, diagnosis, and intervention tools/aids/materials to assist the registered therapist in delivering patient services
  • Assists registered therapists with patient assessments and treatment programs, in accordance with the College of Physiotherapists of Ontario (CPO) and the College of Occupational Therapists of Ontario (COTO) guidelines
  • Promotes patient safety in alignment with the Vision, Mission, Values and Strategic Directions of the HCCSS
  • Utilizes Sensory Tech Platform (and/or other organizational platforms e.g. OTN) to receive clinical support from directing therapists to implement treatment plan
  • Adheres to HCCSS’s patient safety policies and procedures
  • Implement treatment plan with patient under the direction of a registered therapist (PT/OT)


What you must have?


  • PTA/OTA certification recognized by the College of Physiotherapists of Ontario (CPO) and the College of Occupational Therapists of Ontario (COTO) required
  • Graduate of an accredited Physiotherapy Assistant/Occupational Therapist Assistant 2-year diploma program
  • Previous experience as an Occupational Therapy Assistant/Physiotherapy Assistant in a health care setting within the last two years

Attends Health and Safety in-services and takes action to address unsafe conditions and procedures to ensure a healthy and safe work environment
  • Clear understanding of the scope and limitations of the Rehabilitation Assistant’s role and their relationship with the registered therapist
  • Ability to handle complaints from patients, their family members/caregivers/SDMs, and consult with higher authority when appropriate
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel)
  • Ability to prioritize multiple, competing demands
  • Knowledge of equipment, modalities, materials, and intervention processes used in the provision of occupational therapy and physiotherapy services
  • Familiar with rehabilitation principals and clinical terminology used in interventions
  • Relevant experience assisting registered therapists in a community/health care environment
  • Ability to accurately prepare required documentation, forms, and materials for patient records
  • Knowledge of applicable COTO and/or CPO professional guidelines
  • Knowledge of occupational therapy and physiotherapy professions
  • Advanced written and oral proficiency in English
  • Ability to effectively exchange information with a variety of internal and external contacts
  • Maintains confidential and sensitive patient information
  • Ability to operate copy machines, fax machines, and other office equipment
  • Administrative tasks and equipment/supply management duties are carried out with minimal direction
  • Ability to organize and maintain filing systems
