Job Description
Closing the Gap Healthcare (CTG) is dedicated to providing Canadians with the highest level of care across a wide range of home and community settings.



In partnership with the NSH, we are looking for a Client Support Worker to be part of our Bridge Shelter Team. You will be working alongside other healthcare professionals to provide information, referrals, advocacy, goal planning, safety planning, and crisis intervention support as needed.


The Bridge Shelter located at 201-101 Wyse Road Dartmouth NS provides shelter services, healthcare services, and housing support.


Reporting to the Client Service Director, the Client Support Worker utilizes a social justice perspective in a client-centered, trauma-informed, anti-oppressive, non-judgemental shelter environment. If you are comfortable working with and responding to people experiencing a crisis, and those who may be using substances and providing them with the support they need, this may be the role for you.


This is a full-time permanent role located in our Dartmouth location.


  


What You Will Be Doing


  • Maintains accurate client records and is responsible for updating client information
  • Ensure that all interactions with clients are carried out in a safe, dignified, and unobtrusive manner
  • Strives to maintain the safety of the communal environment and protect the rights of all clients
  • Maintains client confidentiality and privacy
  • Provides information, referrals, and support to clients residing on our health services floor
  • Provides regular reports and communicates pertinent information to oncoming staff and our health services team


Skills Needed For Success


  • Computer skills and report writing ability required. 
  • Excellent problem-solving skills and understanding of the barriers that deter at-risk individuals from obtaining and maintaining safe and affordable housing. 
  • Knowledge of issues and resources in the community related to homelessness, poverty, harm reduction, mental health and addiction issues, community development, and capacity building. 
  • Education and/or experience in human services, social work, or related field
  • Good organizational, interpersonal, written, and verbal communication skills. 
  • Strong team player and ability to work independently and with multidisciplinary teams. 
  • Working knowledge of Housing First approaches, the social determinants of health, and services available within the community that impact an individual’s ability and/or opportunity to move beyond personal and systemic barriers.
