Key Responsibilities


Reporting to the Patient Services Manager, the incumbent will be responsible for


  • Sharing information with partners including WRPS following Home and Community Care Support Services policies and legislation
  • Assisting to develop awareness and understanding of issues affecting older adults at risk including elder abuse and the resources available
  • Performing other duties as assigned.
  • Facilitating referrals to purchased and community services and services focused in the area of risk and abuse including The Restorative Justice Program, Victim Services, Victim Witness Assistance Program, and other accredited agencies, to people affected by elder abuse
  • Providing co-ordination, information about services and service options and referral for older adults who are experiencing issues of risk including elder abuse and their families
  • Assisting with the development/implementation and/or enhancement of programs, services or initiatives that address the issue of older adult risk and elder abuse
  • Facilitating a forum for those working directly or indirectly with the issue of elder abuse to meet on an ad hoc or regular basis depending on the needs of the care community
  • Providing consultation and expertise to colleagues and partners supporting older adults experiencing issues of risk including abuse (Domestic Abuse and Sexual Violence Team, Women’s Crisis Services)
  • Conducting ongoing assessments to identify areas of need or gaps in service as it relates to prevention or mitigation of older adults at risk and advocating/initiating efforts to meet these needs in the community
  • Providing an intake and assessment role for older adults experiencing issues of risk and abuse and their families. The Senior Support Consultant assesses for risk including abuse, and including whether or not basic necessities are provided, and what support services might be appropriate to decrease risk of abuse
  • Working with officer(s) from the WRPS to respond, advocate, and provide intervention services to seniors and their families who are experiencing issues of elder abuse including neglect


Education And Experience


  • The ability to work independently in a highly organized manner is required.
  • Fluency in French language preferred.
  • The position of the Senior Support Consultant requires a high degree of independent decision-making in environments characterized by conflict and risk management and a commitment to the principles of restorative justice.
  • This position also requires proficiency in the use of a personal computer in a windows networked environment, using Word and database software.
  • The ability to travel throughout Waterloo Region and Wellington County is required.
  • Proven skills in the areas of risk identification and management, communication, interviewing, complex problem solving and conflict resolution, interpersonal and leadership within a team-based setting is required.
  • Preference will be given to applicants with experience working in a role that focuses on supporting adults experiencing elder abuse.
  • Minimum of two years of community care coordination experience is preferred.
  • Membership in good standing, with the appropriate college, as a Registered Nurse, or membership in good standing with the College of Social Work, with a degree at the Master’s level.


This position may be required to provide relief at other Home and Community Care Support Services locations or teams as needed and may be part of the on call rotation as per the provisions of the ONA collective agreement.
