Job Description
We are currently seeking a Regular, Fulltime Manager Quality and Risk


Postion Summary


Reporting to the Director, Quality & Risk, the Manager, Quality and Risk is responsible for supporting the implementation and oversight of a robust and integrated Quality Framework in collaboration with HCCSS leadership team.


This position requires a high level of independence and is expected to provide the leadership to ensure Home and Community Care Support Services has and follows a robust quality framework, policies and procedures to optimize quality of care, patient safety and to achieve quadruple aim.


The focus will be on supporting the delivery of exceptional care – in alignment with our organization’s values. The work of the Manager, Quality & Risk will be aligned with provincial directions in quality improvement, risk management and patient safety; and involves working with staff at all levels of the organization, as well as with many external stakeholders.


Key Responsibilities


  • Leads or participates on inter-agency committees to improve processes regarding patient care across the continuum
  • Provides support and guidance to staff and leaders to reduce risk for patients, staff and the organization
  • Leads Quality of Care Reviews in collaboration with patient services following patient safety events including incident analysis and identification of recommendations for improvement and facilitates joint reviews with other health service providers as identified
  • Regularly monitors incident reporting system to track, analyze, report trends, identify improvement opportunities and raises issues of serious concerns or risks to the senior leadership
  • Contributes to a safe and healthy environment by following safe work procedures, reporting injuries, illnesses, and unsafe working conditions
  • Works closely with HCCSS Quality & Risk teams to share quality management practices, support quality initiatives and leverage opportunities for integration and collaboration
  • Performs a variety of duties related to managing potential risks and liabilities related to quality of care and patient safety
  • Under direction from the Director of Quality & Risk ensures there is consultation with HIROC and or legal in circumstances where it pertains to quality of care and patient safety
  • Identifies opportunities for quality improvement initiatives/activities to support the adoption of best practices related to patient safety, incident reporting system, processes and practices throughout the organization and with service provider organizations
  • Provides required education/training to advance organizational knowledge of quality improvement principles, methods, tools as well as patient safety, and promotes a culture of patient safety
  • Provides expert advice to leaders on the design and implementation of the patient and safety framework, policies and procedures that ensures high standards of quality of care and patient safety


Skills And Abilities


  • Knowledge of basic statistical analysis, and qualitative problem solving tools
  • Excellent analytical and problem solving skills;
  • Leads or participates on inter-agency committees to improve processes regarding patient care across the continuum
  • Establishes and maintains effective working relationships with managers and staff at all levels to advise on and facilitate quality improvement to enhance patient care
  • Experience with measurement and evaluation
  • Strong planning and organizing skills
  • Works closely with HCCSS Quality & Risk teams to share quality management practices, support quality initiatives and leverage opportunities for integration and collaboration.
  • Ability to use MS Office applications (e.g., Word, Excel, PowerPoint, etc.)

  • Evaluates processes and information from numerous sources, using qualitative and quantitative analysis, to identify issues and develop recommendations for improvement
  • Analyzes organizational barriers in order to develop strategies for introducing change and for building a culture of quality, patient safety, risk mitigation and continuous improvement
  • Adapts leading practices in quality and risk management and quality improvement to rapidly changing HCCSS environment and programs to support delivery of best results


Accountability


  • Position has latitude to make independent decisions regarding data collection tools, monitoring and evaluation techniques, analysis tools, approach to change management, communication and education plans, and project plans, within established organizational frameworks;
  • Monitors information and reports and raises issues of serious concern or risk to the Director, Quality and Risk
  • Responsible for staying informed of new developments and emerging issues in portfolio and providing recommendations for appropriate action to the Director
  • Under the general direction of the Director, Quality and Risk, this position is accountable for delivery of identified strategic objectives for quality embedded in the HCCSS Quality Framework


Education Qualifications


  • Formal training in quality improvement methodologies and framework
  • Project Management certification or designation is an asset
  • University degree in Health Sciences, Health or Business Administration or related field or equivalent combination of education and experience working within the quality and risk environment.
  • Regulated Health Professional is an asset


Experience


  • Knowledge of the balanced scorecard approach
  • Knowledge of quality & patient safety management concepts and experience in applying them in the healthcare sector
  • Knowledge of indicator development, analysis and benchmarking
  • Experience in working with simple database programs such as excel and access
  • Strong relationship management skills with the ability to effectively engage and collaborate with internal stakeholders and health system partners
  • Minimum of 3-5 years of experience in patient safety and/or quality, including 1-3 years in a management role is considered an asset


Physical and Sensory Demands


  • The job requires a limited amount of physical, visual, and auditory exertion, but does not usually produce fatigue or require periods of rest. Freedom of movement exists, and the job does not confine the employee to a prescribed body posture


Visual And Auditory Requirements


  • Ability to read a computer screen for email, preparing documents and reports
  • Reads complex contract language and a variety of reports, including charts, statistics, slides


Working Environment


  • Minimal exposure to disagreeable conditions typical of an office position exposed to stress and pressure associated with multiple priorities and deadlines


Health and Safety


  • Ensure Home and Community Care Support Services and worker compliance of the OHSA, policies, procedures, use of personal protective equipment (as appropriate), training and education for health and safety
  • Demonstrate proactive approach to safety, to reduce risk and seek opportunities for improvement by asking questions and making suggestions related to current policies and processes
  • Attend training required by Home and Community Care Support Services by their supervisor, and apply the instruction and learning to protect themselves, coworkers and patients
  • Report safety concerns, errors, and/or near misses that may put workers and/or patients at risk and participate in corrective action; support and lead investigation into all incidents, injuries and occupational illnesses and complete reporting requirements in a timely manner
  • Use or wear protective equipment, devices and methods as outlined by Home and Community Care Support Services and supervisors and work in a manner that promote the safety of themselves, coworkers and patients.
