Integrated Service Consultant Jobs

The Integrated Service Consultant provides case management services for youth whose needs cannot be met in a single service stream and requires an integrated approach across Mental, Physical, Behavioural and Developmental Health. The Integrated Service Consultant supports children/youth/families who have been deemed eligible for the Mi-Care program by navigating services (both internal and external), and monitoring the progress of the Mi-Care integrated Plan.

• Committed to continuing education and professional development
• Minimum of 5 years experience in case management in working with clients across Mental, Physical, Behavioural and Developmental Health
• Proactive emphasis on risk reduction and problem solving
• Well-developed knowledge of community processes and services
• Must possess valid drivers license, reliable transportation and appropriate level of automobile liability insurance
• Must be able to obtain a valid Vulnerable Sector Screen (VSS)
• Ability to work in a fast paced environment with changing priorities and deadlines
• Superior reporting and communication skills
• Undergraduate Degree: Social Work, Social Services, Sociology, Psychology
• Commitment to quality improvement identification and implementation
• Knowledge, skills and abilities required to support families in accessing services and navigating systems including children's residential facilities

Unit Summary
McMaster Children's Hospital is the regional academic centre for pediatric acute care, rehabilitation, developmental and mental health services. The Mi-Care Program is compromised of MCCSS and MOH funded services for individuals up to the age of 18, who needs cannot be met in a single service stream and requires an integrated approach across Mental, Behavioural and Developmental Health. The Mi-Care Program supports a culture of innovation, family-centred service and evidenced based interventions.
Schedule Work Hours
Monday to Friday, days (evenings and weekends may be included during a crisis)
