Description


Job Summary:


The Social Worker will be responsible to provide information, assessment and entry facilitation into Reena's Community Participation Supports and Residential Systems and to maintain and act as the contact for individuals who have chosen Reena to be their lead agency. The Social Worker plays an integral role in the decision-making process in relation to ongoing planning for individuals already within Reena's system of operations, and intervenes in cases of complex need. The Social Worker provides assessment, case management, clinical and supportive counseling, service coordination, crisis intervention, advocacy, and outreach. The incumbent is further responsible for developing, leading and facilitating psychosocial and therapeutic groups.


The position maintains presence and contribution at a number of high-level internal and external committees to liaise and coordinate services within the Developmental Services Ontario (DSO) scope of authority and within the Ministry of Health and Long-term Care and the Central Local Health Integration Network (LHIN).


Job Responsibilities:


Intake Responsibilities


  • Provides ongoing assessments for individuals seeking or sent to Reena for intake
  • Other duties as assigned
  • Works collaboratively with all stakeholders and departments as needed, to ensure smooth transition
  • Conducts meetings with families and other agencies to plan for intake
  • Produces contracts for individuals entering Reena system
  • Coordination of initial ISP meeting at 3 months following intake into service
  • Prepares all necessary documentation for intake
  • Acts as resource for inquiries into Reena and DSO services
  • Provides ongoing case management and crisis management for families in Reena system


Clinical Responsibilities


  • Provide advocacy support and advocate with individuals and/or their family or caregivers.
  • Provide preliminary assessment of risk in crisis situations, consult and devise an intervention and action plan, and provide crisis intervention.
  • Perform psychosocial and/or psychological assessments.
  • Provide service/care co-ordination, system navigation, short-term and long-term planning and follow-up support.
  • Provide clinical and/or supportive counseling to individuals utilizing a wide range of psychotherapeutic approaches that best meet the individual's needs and circumstances (e.g. Cognitive Behavioral Therapy, Solution Focused therapy, Dialectical Behavioral Therapy, Motivational Interviewing, etc.).
  • Link individuals to various community resources, and facilitate appropriate referrals to other support services as needed.
  • Participate in case conferences and clinical meetings in order to ensure the coordination of services and a continuum of care.
  • Provide mental health outreach functions to the Jewish Community
  • Other duties as assigned.
  • Provide case management services to clients over an extended period of time, through the development and implementation of a care plan to address their priority and emerging needs.
  • Provide virtual and in-person mental health counselling, programs and group sessions
  • Develop and implement service plan and/or coordinated care plans.


Formal Education and Experience:


  • Masters of Social Work (required)
  • 2 years' experience in health and developmental services sectors
  • Experience in performing needs assessment to identify service needs and gaps that can be addressed through psychosocial and/or therapeutic groups.
  • Knowledge of the intake process
  • Ability to quickly develop extensive knowledge and experience within the Jewish Community in the York Region and Toronto
  • Knowledge of the mental health sector is an absolute asset
  • Mental health counselling skills
  • Experience and verifiable knowledge of the community served
  • Experience in dealing with families and counselling modalities
  • Annual renewal & registration with the OCSWSSW/member in good standing
