Reaches out to students with health concerns Provides case management support, advocacy and wellness check-in for students.

Develops relationships with community agencies, including maintaining counselling databases of community resources (availability, wait lists, and eligibility requirements).

Provide support to the counselling team by assisting students with system navigation, community referral, and wellness check-ins.

With student consent, joins counselling sessions to facilitate introduction/warm transfer or to support students waiting EMS transfer.

Connects with external health providers and community agencies to request clinical records, discharge summaries or other required documentation/clinical information.

Meets with students to complete consent forms on behalf of counsellor, nurses and physicians.

Customer management, sales, and corporate partnerships/relations experience

Evaluate staff by providing on-going verbal feedback and periodic, written job performance evaluations as per the organizations Performance Management system

Degree in Business, Recreation Management, Kinesiology/Kinetics or related discipline, or equivalent

Complete additional projects as assigned by the Manager, Service Operations

Demonstrate excellent verbal, written, and oral skills

Possess strong coordination, collaboration, and leadership skills

Ideally a minimum of 2 years of experience in disability management across multiple provinces in Canada;

Actively participate in various absence management continuous improvement projects.

Support the Absence Management Manager with wellness related projects.

Bachelor’s or equivalent degree in a relevant field (HR/Industrial Relations, Management, etc.);

Strong organizational skills, with the ability to prioritize workload, take initiative and manage multiple tasks with little supervision;

Manage administrative aspects of disability cases (opening disability file, coordination with payroll, gradual return, etc.);

Excellent leadership, organizational, team building and time management skills

Assists with application of the technical skills and conducts routine audit to ensure documentation is in line with Company’s standards.

Ensures any gaps in documentation is communicated back to teams and provides re-education to Personal Support Workers and Wellness Nurses.

Minimum (3) years’ experience as an RPN

Strong oral, verbal and interpersonal communication skills

Experience with Quality Improvement Processes and tools is an asset

Change management experience will be considered as an asset

Provides support and coaching to various levels of management and employees on wellness

Experience or knowledge in a similar role
