The Opportunity


Reporting to the Director of Operations, this position will work with or within teams to provide health care services for clients with complex and multidimensional medical, psychosocial, environmental, and instrumental needs. The Social Worker III (SWIII) will provide advanced clinical services, consultation to the interprofessional team and clients/families and supervision of students and team members as assigned. This position may also act as clinical lead providing leadership in the planning, development, delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s). The SWIII will be guided by their education, experience, demonstrated skill level and Carewest Policies and Procedures. The SWIII will play a leadership role in supporting an integrated and holistic approach to client and family centered care, health promotion and maintenance. The SWIII has both departmental and sub-acute program responsibilities. This is considered a multi-site position where travel may be involved. Responsible for other duties as required.


Qualifications & Experience


  • Registered member in good standing with the ACSW. Psychosocial Intervention designation is preferred.
  • Required: 5 years clinical experience in inpatient or outpatient social work role (e.g., acute care, oncology, neurology, medicine, geriatrics, palliative care, and home care). Minimum 3 years experience in mental health. Recent working experience with program development or group facilitation in Transitions and change. Recent working experience in financial assistance. A leadership role in research, program development or service delivery project within the last 2 years. Recent direct clinical supervision of interdisciplinary allied health teams (i.e., team/clinical leader experience). Experience in mentoring, coaching, educating other health care providers. Demonstrated clear pattern of professional/leadership development (e.g., Enhancing Team Effectiveness, Art of Accountability). Demonstrated knowledge/engagement in social work services related to best practices, governance, legislation, ethics, and current issues. Demonstrated ability to build partnerships/ foster effective relationships. Demonstrated ability to identify, manage and resolve conflict. Demonstrated ability to implement/facilitate program and practice change within a complex health care environment. Proficient computer skills/use of clinical information systems (e. g., Microsoft Office, SCM, Connect Care). Preferred: 10 years experience in inpatient or outpatient social work. 5 years of mental health and group facilitation with Transitions and Change. Within the last 5 years, experience as a Leader (Team, Clinical, Manager). Progressive professional development within the last 3 years. Training in Trauma-informed care. Experience outcomes measurement related to program/educational design. Change management education. Experience providing virtual care
  • Valid driver’s license required for travel to other Carewest sites as per business requirements
  • Master’s degree in Social Work
