Job Description

As a Community Occupational Therapist you be responsible for the provision of OT services, to Home and Community Care Support Services HNHB eRehab patients in order to maximize the patient’s function, independence, and safety in their respective environment. Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes over 8,000 regulated health care and other professionals. What will you do?


  • Responds to patient referrals from the Care Coordinator and follows HCCSS HNHB procedures to properly administer eRehab services to patients in a Directing Therapist RoleSeeks informed consent from patients before commencing assessment and treatment plan
  • Teach and assign specific aspects of patient care to a person deemed competent and capable to carry them out and accepts responsibility for these delegated tasks according to standards of the College of Occupational Therapists of Ontario (COTO)
  • Determines patient centered therapy goals and provides treatment program to increase occupational performance, independence, and safety
  • Manage service delivery in conjunction with the Most Responsible Physician, Care Coordinator, patients, their family members/caregivers/substitute decision-makers (SDMs), and other service providers, via virtual models of care (e.g. video, telephone, secure messaging), in person visiting and written correspondence
  • Protects patient’s privacy and confidentiality in accordance with legislation, Home and Community Care Support Services HNHB policies, and standards established by COTO
  • Documents and charts assessment findings, goals, treatment plans, and interventions on behalf of the patient in accordance with Home and Community Care Support Services HNHB policies and procedures, Sensory Tech requirements, and as per guidelines established by COTO
  • Contribute to a safe and healthy environment by following safe work procedures, reporting injuries, illnesses, and unsafe working conditions
  • Participate in program development and quality improvement initiatives to improve patient care
  • Uses sound professional judgment and acts in accordance with all HCCSS HNHB policies and procedures and standards of practice set forth by COTO
  • Ability to utilize electronic databases, including electronic patient records, and documentation platforms.
  • Utilizes Sensory Tech Platform (and/or other organizational platforms e.g. OTN) to provide clinical support to Rehab Assistants who work with the patient directly to implement treatment plan
  • Reassesses patient progress on an ongoing basis, and adjusts treatment program when needed, to ensure that therapeutic goals are achieved
  • Advocate for patients as warranted to identify lack of resources/services for patients and to participate in developing alternative resources within the community
  • Assesses the factors influencing the patient’s ability to function in their environment, using a variety of standardized and non-standardized assessment tools, to determine patient treatment needs
  • Maintain professional competence and knowledge of current practices by participating in professional development and ongoing research
  • Work remotely through Sensory Tech Platform for eRehab services and may require occasional travel throughout the HNHB region as require for patient and community engagement
  • Discharge patients when appropriate and refers patients to other internal or external services if necessary
  • Maintains accurate statistics on direct and indirect patient care, submits records of weekly visits and mileage reports, and performs other administrative duties as required, in accordance with HCCSS HNHB policies and procedures


What you must have?


  • At least two years of recent clinical work experience as a directing occupational therapist required
  • Current Registration in good standing with the College of Occupational Therapists of Ontario required
  • Graduate of an accredited degree program in Occupational Therapy (OT)

What would give you the edge?


  • Relevant occupational therapy experience in a community/health care environment
  • Effective listening, observation, and facilitation skills to effectively assess the patient and provide appropriate intervention
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel)
  • Ability to establish and maintain a wide range of contacts with health care professionals and community organizations
  • Advanced written and oral proficiency in English
  • Comprehensive and up-to-date knowledge of occupational therapy tools, processes, equipment, and assistive technology
  • Refers to professional practice standards and guidelines (COTO), relevant legislation, HCCSS HNHB policies and procedures, and professional associations to deliver occupational therapy services and meet patient needs
  • Ability to deal with demanding interpersonal situations and respond with good judgment and understanding
  • Advanced written and oral proficiency in French may be required in specific job assignments
  • Accountable for the provision of occupational therapy services to patients, their family members/ caregivers/SDMs, and keeps other members of the health care team informed and up-to-date
  • Strong collaboration skills to manage service delivery in conjunction with care coordinators, patients, their family members/caregivers/SDMs, Rehabilitation Assistants and other service providers
  • Knowledge of Quality Based Procedures/Best Practices as per RCA guidelines for Stroke
  • Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capability
  • Knowledge of funding agencies related to the/occupational therapy recommendations
  • Knowledge of Sensory Tech Platform (and/or other organizational platforms e.g. OTN)
  • In-depth knowledge of therapy services and other community resources in the HNHB Region
  • Carries out work responsibilities with minimal direction, exercises judgment, makes decisions, and takes action within legislative framework, COTO standards of practice, organizational goals, and HCCSS HNHB policies and procedures
  • Practical knowledge and understanding of relevant legislation (e.g. regarding the provision of health care services, privacy, health and safety, etc.
  • In-depth understanding of HCCSS HNHB business priorities, objectives, and requirements for patient services
  • Good understanding of the roles of health care professionals for eRehab services
