Clinical Leader Virtual Nursing Station

The Clinical Leader assists the Program Directors and Clinical Managers in the implementation of corporate and program strategic initiatives, and the coordination of day-to-day operations to facilitate quality patient care, education and research. The Clinical Leader embraces change management and communicates effectively with all team members and key stakeholders internal and external to HHS. The Clinical Leader has experience in the successful implementation of the virtual care models. The Clinical Leader participates in human resources management and fiscal management of the virtual nursing team and service delivery model. Clinical Leader participates with meeting necessary reporting requirements and providing updates to relevant stakeholders.

• Baccalaureate degree in one of the Health Disciplines preferred or a combination of other relevant academic background and experience
• Recent (8-10 years) relevant clinical and virtual care practice
• Demonstrated leadership and management skills
• Membership in the relevant Professional College/Association
• Evidence of commitment to continuous education through pursuit of undergraduate studies or comparable educational endeavours
• Experience with EPIC workflows, templates, reports and EMR best practices
• Previous experience and knowledge of virtual care preferred
• Demonstrated creating, coordinating, implementing and assessing delivery of care to patients
• Demonstrated excellence in communication, team-building and the ability to initiate and model positive change
• Demonstrated mentoring, supporting, and coordinating patient care among nurses and other medical team members

Unit Summary

The HHS remote patient monitoring RPM and virtual nursing program (Virtual Nursing Station-Surgical Transitions), supports patients from all HHS sites on their discharge pathway with a virtual care and remote patient monitoring approach. The virtual nurses and administrative team practices from the PROTECT Lab at the Juravinski Hospital and Cancer Centre. The PROTECT Connected Health program utilizes a partnership-driven approach with a dedicated pool of clinical resources to support a research evidence-informed model of virtual care with acute care hospital workflow to prevent, address, and mitigate complications of post-surgical and other patients’ recovery at home and to empower patients as they define their recovery goals through a pro-active and innovative remote patient monitoring (RPM). We are exploring to expand the model to include other patient population such as medicine, oncology, pediatrics and other.
