London, Ontario - Community Based

Too many in our community experience the struggle of chronic homelessness. As a member of our Housing First team, you'll work collaboratively with clients to break the cycle of homelessness by helping them to find and maintain housing, laying a foundation for further recovery, and greater independence.


Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING:


  • What your benefits will be - After a 90-day waiting period, will be enrolled in healthcare benefits plan
  • PSLRTA Notice - This posting characterizes this position as currently non-union. However, please be aware that at the organization on September 29, 2022, a vote was held as a result of an application by the Organization under the Public Sector Labour Relations Transition Act. The result was that employees voted to award OPSEU bargaining rights for all employees. Until such time as a new collective agreement is finalized, current terms and conditions of employment and existing, applicable collective agreements, if any, will remain in force and effect.
  • Your pension plan - Will be enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
  • When you'll work - 70 hours over two-week period, rotating schedule of days, afternoons, evenings and weekends
  • What you'll be making - $22.33 - $25.71 per hour, rate based on proof of comparable, previous experience


HOW YOU WILL BE MAKING A DIFFERENCE:


  • Ensure OCANs and/or SPDATs are completed and reviewed according to policy
  • Identify, promote resolution, and management of all emergency situations concerning clients, staff, and property
  • Provide health teaching about mental illness, addiction, and symptom management
  • Ensure effective communication with Director, Manager, staff team, and external treatment teams concerning each client’s progress
  • Motivate and support clients to work towards achievement of their needs and/or goals
  • Assist and coach clients in the learning and management/achievement of activities of daily living such as personal hygiene, chores, budgeting, socialization, etc. alongside them in their apartment
  • Create or update clients’ Crisis Plan
  • Advocate, refer and connect clients with appropriate services, supports, and resources (formal and informal) based on their needs
  • Support landlords as necessary in conjunction with the client, provision of landlord mediation
  • Professionally liaise with community workers and agencies
  • Crisis prevention and intervention; reducing the utilization of EMS, ER visits, days in hospital, days incarcerated
  • Assess clients’ strengths, abilities, needs and preferences as well as social and environmental factors impacting the client, and any vulnerability and risk factors
  • Support clients to secure housing of their choice
  • Support clients with physical, mental health, and addiction concerns, and liaise with clinical staff
  • Champion each client’s capacity for independent community living
  • Coordinate recreational community outings and activities to enhance clients’ quality of life and further connect clients to their community
  • Assist clients to successfully remain in their apartment, providing prevention and diversion strategies to avoid homelessness
  • Provide intensive support including regular, continuous contact with the client for several months, and periodical follow up for one year
  • Include and encourage clients’ natural supports (e.g. Family, significant friends) in goal planning, clarifying the roles each plays towards each client’s rehabilitation


WHAT YOU'LL NEED TO APPLY:


Vaccination Requirements


CMHA Thames Valley Addiction & Mental Health Services requires that all staff members be fully vaccinated against COVID-19, with the rare exemption of those individuals who cannot be vaccinated due to approved exemptions (medical reasons or religious grounds as recognized in the Ontario Human Rights Code). Proof of full vaccination or approval of exemption will be required; further details will be provided to those candidates moving forward in the recruitment process.


Police Vulnerable Sector Check Requirements

We require the following qualifications:


  • University/college degree or diploma in the social science field (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Demonstrated ability to attend work regularly
  • Access to a vehicle and insurance adequate for transporting clients
  • Valid G Driver’s Licence


We prefer the following qualifications:


  • Crisis intervention and prevention training
  • One year of work experience in the mental health and addiction field
  • Applied Suicide Interventions Skills and Training
  • Knowledge of Psychosocial Rehabilitation
  • First Aid & CPR
  • Experience working with homeless/marginalized populations
  • Knowledge of community resources
  • Knowledge of psychiatric medications
