Job Description
Independent Living Services Simcoe County (ILS) is a leading organization providing support and services to adults wishing to live independently and well in their homes. Through our programs and services, we are able to assist adults living with permanent physical disabilities and high-risk seniors aging at home lead dignified, productive and independent lives.


We are seeking a Service Coordinator and Community Navigator: who is responsible for the duties related to intake, general inquiries, gathering statistics and navigation for all ILS client services. In addition, this position assists clients and their caregivers with the coordination and accessing of appropriate community resources and support to make informed decisions.


Description of Duties:


The Service Coordinator and Community Navigator’s tasks may include and may not necessarily be limited to, the following:


Intake


  • Maintain waiting lists on all approved individuals and conduct waitlist list reviews, as requested, to ensure waitlist list is current.
  • Confirm financial eligibility for clients seeking financial navigation for assistive devices.
  • Respond to all service inquires in consultation with the Manager of Coordination and Planning when required.
  • Process all referrals and determine eligibility and wait list priority according to ILS and Ministry guidelines.


Navigation


  • Work with funding groups and referral sources to identify common information requests and help identify the goals, objectives, and gaps for these community resources
  • Provide clients with appropriate government & community funding sources which may assist in securing the needed information and assist with funding applications
  • Maintain an inventory of community resources & provide referral and information as requested
  • Stay current with available resources and vendors to meet client request related to installation of assistive devices and/or home modifications for accessibility
  • Maintain awareness of the various government and community programs and resources designed to assist seniors and individuals with disabilities to participate safely & independently in their community and within their homes


Assessments and Service Coordination


  • Educate the client on the program goals, principles, and external resources
  • Assist with establishing short- and long-term goals with clients when this need has been identified
  • Conduct initial home assessments by using standardized assessment tools, i.e., interRAI tools and all ILS related documentation
  • Complete a Risk Assessment of the client’s physical environment


Financial & Statistical Management


  • Effectively reports, logs, and tracks all financial contributions in collaboration with the Business Administrator
  • Effectively logs and tracks all department identified department statistics
  • Assist with the collection of program stats


Required Qualifications and Experience:


  • Excellent organizational, problem-solving, analytical and strong research skills
  • Strong team player with an ability to work independently
  • Have access to a reliable vehicle and valid insurance for travel
  • Excellent communication skills (verbal and written)
  • Experience in developing plans to achieve client identified goals
  • Knowledgeable of community resources, services available and legislation that affects seniors and individuals with physical disabilities
  • Be able to pass a Criminal Reference Check including Vulnerable Sector Screening
  • Strong customer service focus, and demonstrates knowledge and sensitive to the needs of adults with disabilities and seniors
  • Graduate of a post-secondary program in health care, social work and or a combination of related work experience
  • Ability to use discretion and maintain confidentiality
  • Strong working knowledge of MS Office software in a network environment, including databases
  • Experience in conducting interRAI assessments


This is a Full Time position working remotely, but will require regular visits to our Barrie Main office as scheduled and will require travel within Simcoe County.


Full Time: Contract Position , 12 Months, with possibility of extension. 35 hours per week, Monday-Friday
