Job Summary & Requirements

The Discharge Planner is responsible to work with the interdisciplinary team to achieve effective clinical resource management which includes the facilitation of discharge planning and patient flow from the acute care setting transitioning safely to a community setting that is most appropriate to the patient's care needs. To liaise with external providers to develop and implement individualized plan of care for patients on discharge. To facility patient flow and change across the organization.

Education

• Post diploma in Gerontology preferred
• Registered Health Professional, Baccalaureate prepared required
• Commitment to professional growth and self-development
• Computer literacy working in Windows environment, Meditech and MedWorxx
• Post diploma education in Case Management, Research Methodology and Data Analysis preferred
• Current BLS required

Experience

• Minimum 5 years clinical experience - multiple clinical settings including medical/surgical, emergency and community preferred
• Discharge Planning experience including case management and utilization management with a solid base knowledge of community resources
