Job Description

The incumbent for this position will have a minimum Bachelor of Social Work Degree from an

accredited university and/or an equivalent combination of experience plus a diploma from a

community college in a related field.


The ideal candidate will be bilingual, fluent in French and English, and will bring a combination of

direct experience in providing case management and programming to at risk populations, along

with an understanding of and ability to relate to the diverse populations within Toronto. The

incumbent will have excellent conflict resolution skills, case management and facilitation skills as

well as familiarity with Cognitive Behavioural Interventions and identification of criminogenic

factors, using Motivational Interviewing and Stages of Change to assist individual intervention

plans.

The focus of work for this position will include responsibility for the development of individual

participant goals and plans, facilitation of workshops geared to personal development and

opportunities for positive change for program participants. You will also be expected to

coach/mentor and provide participants with relevant and evidence-informed service interventions

to promote assets, competency, and resiliency. You will assume service coordination functions

while acting as an advocate and providing information and referrals to program participants.

Other duties include mentoring and counselling program participants to encourage social

inclusion and engaging these program participants to identify issues, needs, goals, aspirations,

skills, abilities, and services needed as well as barriers to accessing these services.

The incumbent will have both knowledge of and experience with employment and educational

challenges in order to address individual barriers to personal growth and to support the

acquisition of skills and assets to manage their personal challenges. The incumbent will

demonstrate the ability to carry out brokerage/advocacy/resourcing functions that ensure

participants have access to programs, services and opportunities needed for healthy development

by building and maintaining relationships with internal and external stakeholders and to

demonstrate appropriate behavior's consistent with CTI’s Mission, Vision, and Values.

The Program Facilitator will liaise with a variety of community resources, referral sources,

colleges, skills training canters and specialized services, community agencies and employers in

facilitating achievement of client goals. You are collaborative, thrive in a fast-paced

environment, and bring creativity to developing current and relevant employment and

educational opportunities for and with program participants.



MAJOR DUTIES


The Program Facilitator shall be responsible for day to day supervision of clients assisting in the

intake and referral function, delivery of the skills development training for program participants,

developing and supporting individual plans and implementing a case management process that

includes assessments, formulation of individual goals, implementation of activities designed to

achieve plans, facilitating personal development sessions, liaising with referral sources,

employers, external resources, and providing on-site support to the participants in the

community.

The Incumbent for this position will be responsible for the following:

1. Intake and Assessment including Individual Planning and Case Management

2. Maintaining case recording on client interactions as appropriate and as per agency

guidelines, leading to overall assessment of client performance and impact of service

3. Co-Facilitation of Personal Development Workshops and Individual Sessions including de-

briefing and compliance with internal/external reporting requirements

4. Outreach to engage potential participants and employers/business to secure partnerships for

participant placement experiences. Developing and maintaining a broad base of diverse

partnerships, including community partners and businesses

6. Assigned Other Duties in Accomplishing Goals of the Program

7. Ensuring accuracy, confidentiality and safekeeping of agency records

8. Participating constructively in clinical supervision with the Manager of Programs and

exercising due diligence with respect to self-care and accountability for practice


The Program Facilitator will work collaboratively with team members and will be under the

direct supervision of the Manager of Programs.

Although the majority of the work will take place between 9 a.m. – 5 p.m. Monday to Friday, some

evenings and/or weekend work may be required. Salary Range: $50,000 to $60,000 prorated based

on experience. A valid Ontario driver’s license with access to a reliable vehicle for local travel is

preferred. A Vulnerable Sector Screen (VSS) will be required
