Program Overview – WoodGreen Spadina Housing, Housing & Homelessness Services


The WoodGreen Spadina program serves clients with significant mental health challenges who have experienced homelessness. This holistic program also offers case management, community development, wellness care and coaching, and pest control & hoarding management. The clients live in WoodGreen owned housing small-scale apartment buildings, as well as scattered units throughout the city.


What You Will Do


  • Establish and maintain an ongoing flexible and supportive relationship with clients.
  • Assist tenant/client to develop necessary skills to live in the community (e.g. cooking, housekeeping, budgeting, shopping, leisure planning, etc.).
  • Work in collaboration with other WoodGreen staff to ensure the program operates efficiently and effectively.
  • Identify deterioration in mental status or in independent functioning of client and take appropriate action.
  • Introduce new tenants to other tenants and staff and to the various committees and activities they can participate in.
  • Assist tenant/client to develop a supportive network.
  • Perform initial and ongoing needs assessment from a strengths-based client support approach.
  • Apply ethical standards related to the provision of community support.
  • Keep documentation and client files current and updated and collect data as required.
  • Provide support and/or assistance, if necessary, when tenant moves in/out of their unit.
  • Assist tenant/client to set objectives and plan activities to achieve goals.
  • Provide tenant/client support to empower the individual and increase their independence.
  • Provide crisis prevention, management, and intervention support including conflict mediation, interpersonal support, and assistance in illness management.
  • Document any significant problems, events, changes to services, special needs, etc.
  • Participate and represent the organization at external committees and forums, etc.


What You Bring to the Team
  • Experience in case management, supportive counselling, conflict/crisis mediation, and client engagement.
  • Bachelor’s Degree in Social Work.
  • Experience working with individuals with mental health, substance use/addiction issues, physical health issues, and/or street involvement.
  • 5+ years of solid community experience.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers, or College of Registered Psychotherapists of Ontario, or an equivalent.
  • Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e., Medical exemption).

  • Demonstrated information and referral skills and ability to work collaboratively with service providers and stakeholders.
  • Commitment to a client-centered approach and a demonstrated positive attitude.
