Job Description
Come work as a Community Care Assistant with Vancouver Coastal Health (VCH)!


Vancouver Coastal Health is looking for a Community Health Worker to join the Acquired Brain Injury Transitional Housing Program (ABI THP) in Vancouver, BC. Apply today to join a new, innovative, and exciting Community Health Worker position! You will support a regional ABI Transitional Housing Program that has been co-designed in collaboration with specialists from across VCH, including brain injury, rehabilitation, substance use, mental health and safety. This is the first program of its kind in BC - one that provides a healing, client-driven (“meeting the client where they are at” and “nothing about me without me”), transitional location for people after their acute brain injury and hospitalization. As part of an interdisciplinary team in a community program you will care for clients throughout their stay including transitions and outreach to the community. The ABI program also emphasizes a supportive and innovative environment to learn and expand your knowledge and expertise. As the Community Health Worker you are passionate about helping those most impacted by the housing and toxic drug supply crises and working within a team-based care environment that emphasizes trauma informed, harm reduction and culturally safe care. As a Community Care Assistant at Vancouver Coastal Health you will


  • Assist clients with varied levels of care, observes, and reports significant changes in the client’s presentation.
  • Be passionate about helping those most impacted by the housing and toxic drug supply crises and working within a team-based care environment that emphasizes trauma informed, harm reduction and culturally safe care.
  • Motivate, encourage, and involve clients in their care.
  • Care for clients throughout their stay including transitions and outreach to the community.
  • Collaborate with the health care team, clients and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Be part of a new program that will provide hope at a critical moment.
  • Emphasize a supportive and innovative environment to learn and expand your knowledge and expertise.
  • Provide input and feedback in the care planning process to the health care team.


Education & Experience


  • Experience interacting with persons with dementia, geriatric and/or palliative population will be an asset.
  • Valid BC Drivers License. Local travel requires the use of a personal vehicle.
  • Graduation from a recognized Health Care Assistant Program.
  • Current/Active Registration with BC Community Health Worker and Care Aide Registry.


Knowledge & Abilities


  • Demonstrates correct use of body-mechanics and knowledge of injury prevention.
  • Home management skills.
  • Communicates effectively and in a culturally safe manner with client, caregivers and health care team.
  • Ability to organize workload priorities within a team environment.
  • Demonstrates proficiency in verbal, nonverbal, and written skills through a variety of communication methods (e.g. paper and/or electronic chart, telephone and in-person).
  • Physical ability to carry out the duties of the position.
  • Knowledge of Gentle Persuasion Approach Techniques (GPA)
  • Ability to motivate clients and encourage progression through therapeutic goals
  • Ability to take direction from the health care team and clients
  • Basic computer literacy to operate a computerized client care information system.
  • Basic knowledge of client-centered care and promoting independence
  • Ability to respond effectively and appropriate in a workplace setting.
  • Basic knowledge of self-care and motivational support
  • Knowledge and ability to operate mechanical lifts, following safe work procedures
