Previous dietary experience preferred in LTC (Long Term Care) Home 0-1 year(s).

Clears plates and trays, loads, and operates dishwasher. Cleans pots, pans, and cooking utensils as per procedure.

Participates in proper storage and labeling of dietary supplies and food, including leftovers.

Adheres to established departmental policies, procedures regarding quality assurance, fire, safety, environment, and infection control.

Strong assessment, decision-making, and case management skills.

Good time management skills, with the ability to work independently and co-operatively in a busy and fast-paced multidisciplinary environment.

Previous case management experience in a health care setting.

Demonstrated ability to use chronic disease management principles to empower patients to self-manage their conditions.

Use excellent problem solving and de-escalation skills to mediate issues and care concerns brought forward by patients, caregivers, or service providers.

2+ years of recent experience in community health or a related field.

Good time management skills, with the ability to work independently and co-operatively in a busy and fast-paced multidisciplinary environment.

Pain and symptom management needs of palliative care patients.

Minimum 5 years of relevant experience in community health or a related field.

Training/certification specific to palliative care (i.e., Fundamentals, LEAP, CAPCE, etc.).

Strong assessment and decision-making skills.

Previous administrative experience in an office environment (e.g., knowledge of the formal and informal protocols and methods of supporting senior-level positions)

Post-Secondary Certificate or Diploma in Office Administration or equivalent experience
