OPWZ Long Term Care Veterans Unit

Completion of a Nursing Assistant program and familiarity with basic computer skills and functions.

Communicate effectively in English both verbally and in writing. Must have excellent telephone communication skills.

Previous experience as CNA II or CNA I in Hospice, home health, Facility based care, or oncology.

High School diploma or equivalent.

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

C.N.A Float (Home Care/Long Term Care) for the Burlington service area.

Create and oversee Resident- and family-centered Care Plans; establish personalized treatment goals in collaboration with the Resident/Family

Assign tasks to Physiotherapist Assistants; supervise Resident treatment and progress

Participate in continuous quality improvement program, including measurable outcomes, satisfaction feedback, and quality improvement planning

Eligible for registration or Registrant in good standing with the College of Physiotherapists of Ontario

At least two (2) years of clinical experience; experience in a health care setting is preferable

Extensive knowledge of community resources and maintenance of good working relationships with community partners

Strong assessment and interviewing skills

Bachelor’s Degree in Social Work (BSW) from an accredited and approved School of Social Work/University Program.

License to practice from the Nova Scotia Association of Social Workers

Ability to take initiative and work independently

Previous unit clerk experience preferred

Grade XII plus one year related clerical experience or graduate of a recognized one year secretarial/business program

Typing and computer skills required

Previous experience with STAR an asset

Delegate clinical responsibilities to qualified staff in accordance with Chartwell policy and CNO standards of practice;

A sound knowledge and background in geriatric, rehabilitation or long term care nursing including activation, nursing, and personnel administration;

Must possess good communication, leadership and interpersonal skills.

The responsibilities of this role include but are not limited to

Assess and reassess the resident health status and communicate resident care needs and changes in resident status to key resident contacts;
