Independently manages work plan for portfolio

Develops capabilities of transition leaders within stakeholder community to advance and support CM objectives.

Additional responsibilities on a project may include event coordination, training, writing and/or editing program communications.

Offers ideas for improving work and team processes.

Develops working understanding of future state functional solution, to serve as an informed resource for Transition Network members in community units.

Works with Comms and Engagement teammates to support IRP material development. Develops/tailors program materials to local faculty/unit.

Experience as a Clinical Dietitian in a clinical and/or continuing care setting is required

Experience working with the following programs: Filemaker, MealSuite and Epic is required

Graduation from a recognized degree program in Foods and Nutrition;

Completion of recognized internship program

Registration with the College of Dietitians of Alberta required; Registration with Dietitians of Canada an asset

Understand and worked with the AHS diet guidelines

Competency in dysphagia management, enteral and parenteral nutrition

Demonstrated participation in continuing education and upgrading of skills

Minimum one (1) year working experience as a dietitian preferred

Inpatient clinical nutrition experience an assets

Experience counselling patients in an outpatient setting

Demonstrated interpersonal and communication skills

Knowledge of clincal theory and practice within a client/family centred model for care

Knowledge of enteral and parenteral nutrition, feeding tube access and delivery methods

Knowledge of the metabolic basis of nutritional care

Knowledge of assessing clients with multi-system nutritional problems

Knowledge of other health care disciplines and their role in client care

Knowledge of research process and methodology

Experience with recipe and menu management software

Knowledge / Skills / Qualifications

Leads Cycle Menu Management (CMM) demos to cycle menu customers.

Maintains knowledge of current trends and developments in the field through networking and education events.

Shares and disseminates knowledge across GFS business units and divisions to improve best-practices.

Excellent written and verbal communication skills

Apply knowledge, skill and clinical experience to assess, develop and evaluate nutrition care plans and monitor outcomes.

Coordinate the delivery of clinical nutrition services to a resident population with varying physical and cognitive disabilities.

Provide counseling and education to residents that facilitate optimal nutrition intake and support health maintenance.

Comprehensive knowledge of Long Term Care and gerontological nutrition

Knowledge of swallowing disorders and related nutritional issues related to disability/degenerative diseases.

Demonstrated experience with utilizing various wordprocessing, spreadsheet and nutrition-related computer applications.

Manage and comply with administrative duties as required by the company, as well as professional and legislative requirements

Provide one on one nutritional counselling and education to clients based on individual needs and objectives

Proven ability to apply evidence-based nutrition knowledge to actionable nutritional assessments, plans and coaching

Knowledgeable and adaptable to working with diverse clients from varying backgrounds and cultures

Strong interpersonal skills with patients and colleagues

Excellent oral, written, communication, problem-solving and decision-making skills
