COMPENSATION: $52,535 - $63,260 Annually Prorated

To help fulfill its mission, LOFT is hiring a Psychogeriatric Case Manager.


JOB SUMMARY:


The Psychogeriatric Case Manager provides case management services to vulnerable older adults and seniors living in an Enhanced Specialized Assisted Living Program. Provides case management services to support the physical, cognitive, and mental health needs of vulnerable older adults and seniors. A key component of the position will be the building of a support network of community resources, especially supportive housing providers, which will empower the individual and increase their independence, dignity and self-respect in a secure community environment thus allowing them to move on to live in the community with appropriate supports. There may be occasional weekend and evening hours.


WHAT YOU WILL DO:


  • Act as a liaison and client advocate among those involved with the client’s care including family, other caregivers, medical and other multi-disciplinary providers.
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners.
  • Conduct assessments such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
  • Assist clients in the development and implementation of a care plan tailored to the client’s needs and issues designed to improve their quality of life. Identify and refer to resources specific to those needs and issues.
  • Facilitate connections with primary care and community support services.
  • Provide crisis management/intervention and assist in the development of crisis plans for clients.
  • Attend and actively participate in all required training.
  • Provide supportive counselling or referral and linkages to health care, programs or service providers.


WHAT YOU BRING:


  • Ability to work effectively as part of a team and interact with workers from a variety of agencies.
  • Understanding of cultural competency and anti-oppression framework.
  • Completion of a degree in social services/human services or equivalent.
  • Good oral and written communication skills and computer literacy, including MS word and Excel.
  • Creativity and flexibility to approaches.
  • Experience in group counselling related to mental health, addictions and related issues.
  • Valid drivers’ license, ability to drive work vehicles to transport clients
  • Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills.
  • Counselling, responding and assessment skills.
  • Knowledge of other languages is an asset.
  • Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs including mental illness, addictions and severe social isolation.
  • Reflective of a multi-barriered group, as serviced by the program, a strong asset.
  • Knowledge of community resources and support.
  • Completion of Gerontology is an asset.
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities.
