Job Description
Personal Support Worker Return of Service (PSW ROS) Grant up to $5,000 for a 6-month commitment


Sign-on Bonus - $750


CarePartners is one of Ontario’s largest, most reputable and well-established accredited home health care providers, providing nursing, personal support, home support, palliative, therapy and rehabilitation support services for patients of all ages across urban and rural Ontario. We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 21 nursing clinics.


We continuously innovate to meet patient needs with specialized (award-winning) care programs and technology, including virtual care and Artificial Intelligence (AI).


CarePartners is embarking on a new program in partnership with the Home and Community Support Services (HCCSS) Central East (Durham Region) and we are looking for community-based support workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

In this exciting staffing model, HISH CSWs will have consistent hours of work, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.


The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends


Two-week Rotation


Mon/Tues/Sat/Sun


0800 - 2000 hrs


Wed/Thurs/Fri


0800 - 2000 hrs


This is an equivalent of seventy-seven (77) hours per bi-weekly pay period! Opportunity for guaranteed hours regardless of patient load.


We are looking for Community Support Workers (CSWs) to help us bring wrap-around patient-centered care to the Durham Region Area. Are you someone we should meet?


  • Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices
  • Opportunities to volunteer in countries with limited access to healthcare services
  • Contributing as a valued member of the interdisciplinary care team
  • Paid orientation and hands-on mentoring from experienced, caring staff
  • Experience working with patients with significant and complex care needs such as dementia and mental health
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety
  • 77 hours bi-weekly pay
  • Community Support Worker (e.g.) Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program) education, with preference given to those with a PSW certificate and/or equivalency
  • Ability to work both independently and as part of a team

  • Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions
  • A valid driver’s license and vehicle for which you are insured
