To learn more visit www.clevelandclinic.ca.*POSITION SUMMARY:*Reporting to the Director, Corporate and Community Health Solutions, the role of the Occupational Health Nurse is to:* Provides support for client’s approach to Short Term Disability (STD) and Long Term Disability (LTD) programs* Consultation on case management approach with client’s supplier(s) as required* Participating in complex case review process (conference calls/discussions) with internal and external stakeholders* Support the STD appeal process as required including review of medical information, consulting with internal and external stakeholders, and providing inputs and recommendations to decisions* Evaluating and providing opinion on employee capabilities, level of care, and accommodation needs through in-person consultations and/or directly contacting the employee’s health care provider* Participate in monthly shared learning case presentations with STD administrator to encourage ongoing adoption of client’s best-practice disability management approach.* Review complex workplace accommodation requests and associated case and medical information a) Collaborate with key stakeholders for recommendations on reasonable workplace accommodation solutions b) Meet with employees (in-person or virtually) to understand their barriers and make recommendations on reasonable workplace accommodation solutions* Review environmental reports and together with key stakeholders provide feedback to be shared with the workplace to address any issues or concerns* Attend meetings (in-person or virtual) and otherwise engage with the workplace to address any issues or concerns*RESPONSIBILITIES:** Together with key stakeholders, improving patient outcomes along the continuum of care through the facilitation of evidence-based or best practice guidelines, by intra-disciplinary and interdisciplinary collaboration, by participation in research, by the provision of leadership and through education of patients, staff and others* Together with key stakeholders in the development, implementation and evaluation of relevant procedures, policies, guidelines and standards of care that support patients and their families* Work collaboratively with physicians in the follow up of test results to ensure quality care* Other responsibilities as may arise*MINIMUM QUALIFICATIONS:** Registered Nurse and in good standing with the College of Nurses of Ontario; Certified in Occupational Health Nursing, COHN(C) ®* Current CPR/First-Aid Certificate* Advanced communication skills* Ability to manage less than optimal patient experience issue* Excellent decision-making and problem solving skills* Excellent time management skills* Ability to function collaboratively within a multi-disciplinary team and/or independently* Patient oriented, with the ability to effectively work with diversity and appreciation that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand* Computer and technical skills requiredJob Types: Part-time, Fixed term contractContract length: 12 monthsBenefits:* Dental care* Extended health care* Paid time off* Work from homeExperience:* Occupational Health Nursing: 3 years (preferred)Licence/Certification:* Registered Nurse (RN) License (required)* Occupational Health Nursing, COHN(C) ® (required)Work Location: Hybrid remote in Toronto, ON M5J 2T3
