About The Opportunity
Nova Scotia Health has established a new Care Coordination Centre that will improve how we coordinate the movement of patients throughout their healthcare journey. It will use artificial intelligence, predictive analytics, and patient status to provide a real-time view of access to care, patient flow, and health system resources to support decision making. It will also generate actionable insights to drive continuous improvements in how healthcare is delivered to improve the quality of patient care. The Care Coordination Centre supports teams to identify issues and find solutions together, using real-time data. This will translate into improved patient access and flow, efficient bed management and discharge planning, and more collaboration across health zones in the province. Each regional hospital across Nova Scotia Health will have a C3 Hub which as Manager of Patient and Access & Flow you will provide leadership and decision making in support of access to patient care and transitions. Patient Flow Manager will also be responsible for acute care social work.
About You
We would love to hear from you if you have the following:
  • Competencies in other languages preferred. French an asset
  • Demonstrated knowledge in current trends in health care practice
  • Clinical experience with utilization and patient flow management practices, methodologies or equivalent knowledge, experience and education is required
  • Knowledge of and experience with change management principles and methodologies
  • Relevant Bachelor’s Degree required (BA, BBA, B.Sc. or Health Related Discipline); Nursing preferred
  • Flexibility and ability to work independently as well as a member of a multidisciplinary team
  • Minimum of three years of progressive administrative experience in health care management and/or related experience
  • Relevant Master’s Degree and asset
  • Knowledge of Administrative Policy/Procedures
  • Ability to interpret and execute administrative directives in order to act as the executive representative
  • Demonstrated knowledge of Best Practice Utilization Management and Clinical Flow practices
  • Demonstrated organizational skills in leadership, communication, decision-making, problem solving and motivation
  • Demonstrated effective decision making competency related to patient care management both with respect to engagement of clinical teams and physician leaders as well as administrative clinical leadership
  • Must possess the ability to take leadership role and make independent decisions in any situation, assuring legal and ethical issues are considered and demonstrating diplomacy, tact, creativity and good judgement in interpersonal relationships in a demanding fast paced environment

Permanent Full Time; 75 hours bi-weekly
Compensation and Incentives
$43.91 - $58.34
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
