AuthoraCare Collective is currently seeking a Hospital Nurse Liaison (RN) Float. This is a full-time (40 hours per week) position with week-end responsibility supporting our Greensboro and Burlington campuses/service areas.

Job Summary
Evaluate the clinical needs of newly referred and hospitalized hospice patients. Identify and inform patient and family about appropriate resources. Work with the patient/caregiver and appropriate providers to establish patient/caregiver care goals and identify and implement interventions. With attending physician and selected members of the IDT, ensure that a medical plan of care is established and implemented for the patient. Communicate effectively with other members of the interdisciplinary team and health care providers, as needed.
Education/Experience

• Experience using electronic patient documentation is desired.
• One year of experience in an acute, hospice, home health setting, or long-term care setting plus.
• Currently in good standings with NCBON.
• Currently licensed as an RN in North Carolina

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
