Program Name: Home Support Program


Number of Hours Bi-Weekly: 60


Work Schedule: Days, Evenings, Weekends

Job Summary


The Home Support Worker (HSW) provides support services to clients and families within the framework of VON policies, procedures, and standards. The HSW collaborates with members of the health care team in meeting the needs of clients to attain an optimal level of independence and well-being. Home Support Workers do not perform client-specific delegated tasks


Key Responsibilities


  • Provides respite care to clients as required
  • Performs tasks reflecting the needs of the client and their household’s safety, which may include personal care, light cleaning, preparation of nutritious meals, laundry, and shopping. Observes and listens to the client and caregiver to identify whether the family’s needs have been met
  • Implements approved changes to the care plan under the guidance of the Manager/Team Lead
  • Functions and reports in accordance with VON's policies, procedures, practices and guidelines designed to promote and protect client safety
  • Other duties as assigned
  • Contributes to the development of the personal support care plan by reporting any observed changes or needs to the Manager or Team Lead


Education, Designations And Experience


  • High school diploma or GED
  • Experience working with elderly or ill individuals and/or children
  • Household management experience
  • Three years demonstrated experience providing support services within community healthcare
  • Certificate in Home Support Worker (Level I, II, III) is an asset
  • CPR level C Certificate required

  • A current and original copy of a satisfactory Criminal Records Check
  • Must be able to wear Personal Protective Equipment (PPE)
  • A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required
  • Must have access to a personal vehicle for work and possess both a current driver’s license and proof of vehicle insurance for business purposes


Work Conditions and Physical Capabilities: Attention to detail; Lift and carry using appropriate lifting techniques; Bend, push, pull, grip, reach, kneel, crouch using good body mechanics; Walk, sit, stand, climb stairs; Hand movements.
