Full-Time, 35 hours/week, 1 year Contract with possibility of extension
POSITION: Crisis Response Network Coordinator - Social Services
LOCATION: Barrie with travel throughout Simcoe County-Hybrid Work Model
PURPOSE OF POSITION
To coordinate and monitor crisis response services within Simcoe County for individuals with a developmental disability with mental health needs and/or challenging behaviours, and/or involved or at risk of involvement with the criminal justice system, who are experiencing a psycho-social or situation crisis.
Statement Of Major Responsibilities
Service Delivery
  • To support eligibility for services as required.
  • To provide timely and appropriate crisis response to individuals and their families and/or caregivers who are requiring crisis support.
  • To develop a person-centered, individualized approach to service.
  • To complete assessment of situation and risk, identify service and safety needs, develop intervention and support plans and complete appropriate referrals to facilitate crisis resolution.
Service Coordination
  • To receive ongoing clinical consultation from the Central East Network of Specialized Care (Community Living Huronia) Regional Coordinator.
  • To build, maintain and enhance formal inter-agency collaborative service agreements with all community service partners.
  • To work with Central East Network of Specialized Care (Community Living Huronia) Regional Coordinator, other Quadrant Coordinators and Network partners to develop and implement quality assurance tools, client and service data collection and tracking that will evaluate and monitor the use of services being provided by the Crisis Response Network.
  • To represent the Quadrant Crisis Response Network at Simcoe Resource Management Committee, Mobile Resource Team, and Regional Crisis Committees.
  • To ensure continued collaboration and shared responsibility for support/involvement by developmental services and mental health agencies.
Organization
  • To record and report client information as necessary for effective service delivery and case management and prepare progress notes.
  • To adhere to all agency standards, policies, and procedures
  • To collect and report service delivery data as required. Computer/keyboarding skills are required.
  • To regularly attend and participate in supervision and engage in annual performance appraisals.
  • Must be aware of and adhere to Health & Safety Policy Rules & Regulations.
  • To participate in staff training as recommended by the manager and demonstrate initiative in learning and developing new techniques of service delivery.
To carry out other such duties as may be assigned by the manager that are consistent with the incumbent’s position
Qualifications
Education and Experience
  • Post-secondary Degree at a University Level in one of the Social or Behavioural Disciplines with directly related experience (3 to 5 years) working with persons with developmental delays and mental health issues or an acceptable equivalent of training and experience.
Knowledge and Skill Level
  • Excellent communication and negotiating skills
  • A working understanding of French Language is beneficial.
  • Knowledge of community resources for individuals with special needs, proven ability to work effectively on committees, seek out and develop partnership opportunities and formalize service agreements, and relevant legislation
  • Ability to demonstrate sound clinical judgement with respect to complex client situations.
  • Excellent interpersonal and organizational skills, demonstrated ability to self-manage time and multiple tasks
  • Knowledge of computer applications
Valid driver’s license and access to a reliable vehicle
This position is a Non-Bargaining Unit position

Hourly Rate Range $28.61 to $32.32 per hour.
