COMPENSATION: $54,488 - $70,050 Annually Prorated

To help fulfill its mission, LOFT is hiring a Coordinator of Care.


JOB SUMMARY:


The Coordinator of Care Worker will provide support to clients of the LOFT Hub and Spoke – Penetanguishene program and the Back to Home – Penetanguishene hub (7 Harriet St.). The candidate will be responsible for the intake and coordination of assisted living/supportive housing services to 40 older adults and seniors living within Penetanguishene. The position serves a diverse client population which includes frail, cognitively impaired seniors who may be living with mental health and other challenges while living in the community. The staff will provide care coordination, assess clients for service eligibility utilizing the InterRAI CHA assessment and review collaboratively with Program Manager, the assisted living services to ensure the program meets the needs of the client within the mission, vision and values of LOFT’s program.


Note: Staff will be required to work in residents’ homes and may therefore be exposed to second-hand smoke. In addition, this position may require weekend shifts.


WHAT YOU WILL DO:


  • Providing direction and support to staff related to direct service delivery.
  • Re-evaluating the level of support provided and assessing client’s suitability for independent living;
  • Providing 24 hour crisis management on a rotational basis through a pager or similar system;
  • Participating on case conferences and providing case management services to those who require these services;
  • Establishing and maintaining a service referral network specifically geared to meet the needs of clients;
  • Maintaining accurate records and files on the provision of service to residents/clients;
  • Establishing and maintaining a support network of social, family, medical and psychiatric community links and resources; advocating on their behalf in navigating the social services and health care systems to ensure accessibility and availability of services
  • Developing a plan of support and recovery which includes identifying the priorities, goals and required supports for them to stay home in the community, in conjunction with other professionals and staff;
  • Promoting the personal empowerment of each client through coaching, mentoring and support, enabling each to meet the daily requirements of their living environment and directly assisting with their daily living activities, as required
  • DIRECT SERVICES
  • Mediating issues or conflicts between clients and neighbours, teaching conflict resolution; following-up on issues with residents to ensure resolution.
  • Developing and implementing operating procedures for the program and ensuring compliance with established standards;
  • Assisting with and ensuring assessments (e.g. RAI CHA) are up to date and completed
  • Ensuring the timely collection of all required data (CIS) and that these reports are distributed to LOFT by the required dates;
  • Liaising and acting as the main linkage with other service providers as needed;
  • Being involved in assessing the individuals referred to the program, determining their daily living requirements, including personal care and nursing needs;
  • RECORDS AND DATA COLLECTION
  • Maintaining a general awareness of the daily activities in the program, identifying staffing issues;

  • Excellent assessment and crisis intervention skills, with well developed problem solving abilities and interpersonal skills;
  • Completion of a certificate or equivalent in Human Services (i.e. Social Service Worker, Social Worker, etc.); with eligibility for enrolment in the required regulatory body of the province of Ontario
  • Identifying gaps in service and recommending solutions;
  • Assumes other duties at the hub as appropriate
  • Starting vacation time above minimum standard,
  • Proficient in InterRAI CHA assessment as well as applicable computer programs; experience with OCAN (Ontario Common Assessment of Need) an asset
  • Develop and coordinate training and professional development programs/activities
  • Experience with frail, cognitively impaired older adults and seniors with complex challenges, incl. mental health, physical/medical, cognitive impairments, severe social isolation, etc.;
  • Develop cooperative action, build staff relationships and act as a liaison between staff and clients

  • Excellent knowledge of services available from the HCCSS, community support providers and psychogeriatric supports;
  • Recommend and liaise with the Program Manager in the selection of all staff on the basis of job description and salary scales by LOFT Community Services guidelines
  • Coordinate and attend regular staff meetings and supervisory meetings as required
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities;

  • Ensure that LOFT Community Services policies and procedures are understood and reviewed annually by staff and are carried out by them, in collaboration with the Program Manager
  • First Aid and CPR certification;
  • A minimum of three (3) years or care coordination/case management experience;
  • Participating in the annual program planning process which includes preparing the service plan, operating plan, client satisfaction feedback process, and other research activities;
  • Ability to communicate in a different language, an asset.
  • Assisting in the selection, orientation, training and performance management of staff
